Just finished talking with our eDMR IT administrator and all four facilities should be under one owner. The only way for
us to separate them is if each facility was listed as its own business in NCSOS.
Information I'll need:
• Address to use for Corning Optical Communications LLC. According to NCSOS, the mailing address is 4200
Corning Place, Charlotte NC 28216. Documents relating to the permit will not be mailed to this address.
• An individual that is legally responsible for all four facilities (maybe someone from corporate), please make sure
to include their name, email, phone number, and mailing address.
• I'll give each plant manager delegated signature authority and list them as permit contact, that way, they will
receive the mail regarding their permit. Please provide their name associated with each permit number, their
phone number, email, and mailing address.

• I'll give each plant manager the privileges to certify and submit for eDMR
