This letter is to inform you of a design change to the referenced project at Impact Site S-
2. The change is requested due to landowner concerns and to prevent debris build -up in
the culvert. The original plans call for an aerial crossing of UT -Zacks Fork downstream
of an existing 36" CMP; the new plans show this aerial crossing being shifted to the
upstream end of the same pipe. The shift will require 10 linear feet of the 36" CMP to be
removed and a new headwall will be installed. Stream banks, adjacent to the removed
pipe section, will be graded to match existing undisturbed banks immediately upstream
and will be stabilized with coin matting and seeded with native riparian seed mix. The 10
linear foot section of stream bed will be stabilized with riprap. The old sewerline pipe
downstream of the culvert will be cut flush with the stream banks and permanently
plugged. There will be no additional impacts at this location.
The original permit authorized 35 linear feet of impacts at this location. The revision will
result in total impacts at this location of no more than 20 linear feet. Attached are copies
of the original and revised plans showing the changes to Impact Site S -2.
