A letter of request for an Authorization to Construct was received October 6, 2014, by the
Division of Water Resources (Division), and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of
modifications to the existing 40 MGD T. Z. Osborne WRF, with discharge of treated wastewater into
the South Buffalo Creek in the Cape Fear River Basin.
This authorization results in an increase in design and permitted capacity to 56 MGD, and is
awarded for the construction of the following specific modifications:
Demolition of existing traveling bridge tertiary filter components and retrofit of six (6) new
diamond cloth filter systems into the existing filter concrete structures, each system filter area
is 2,560 ft2 nominal 10 pm and is to include backwash pump and other filter support features,
specified filter system spare parts, piping, and controls; installation of one (1) new chlorine
contact chamber with dual 47.2 ft x 52 ft basins, influent channel, post aeration cascade,
effluent channel, parshall flume, single influent line liquid chlorination injector/diffuser, single
submerged liquid sodium sulfite solution diffuser, dual 600 GPM at 42 ft THD 15 HP
submerged drain pumps (one per basin), piping, and controls; demolition of existing specified
liquid chlorination feed system and installation of five (5) new 80 GPH at 30 psi THD liquid
chlorination solution metering pumps with direct DC motor control, piping, and other controls;
demolition of existing liquid sodium sulfite feed system and installation of four (4) new 10
GPH at 30 psi THD liquid sodium sulfite solution metering pumps with direct DC motor
control, piping, and other controls; demolition of existing PACL solution metering system and
installation of six (6) new 120 GPH at 30 psi THD PACL solution metering pumps with direct

DC motor control, drip systems, piping, and other controls; installation of one (1) new 36 ft
diameter scum pre -concentrator with mechanical rakes/baffles, surface sprays, overflow weir
ring, one (1) 200 GPM at 14 ft THD 2 HP drain pump, two (2) 150 GPM at 30 psi THD 10 HP
pumps with VF drive, one (1) 4 inch inline grinder 150 GPM 3 HP, piping, and controls;
upgrade of (6) existing primary settling tanks scum pumps, four (4) 400 GPM at 20 ft TDH 5
HP, two (2) 330 GPM at 14 ft THD 5 HP, piping, and controls; installation of new 54 inch
diameter gravity flow outfall line from existing junction box, adjacent to existing 54 inch
outfall line, including modification of the existing outfall structure and NPW piping to
accommodate second discharge pipe; in conformity with the project plans, specifications, and
other supporting data subsequently filed and approved by the Department of Environment and
Natural Resources.
This is the final phase (Phase III) needed to complete requirements to operate a 56 MGD
facility. Engineer Certificates for 047384A03 (Phase I) and 047384A04 (Phase II) must be
received by the Division indication completion of all approved items in conjunction with the
final Engineer Certificate for this permit before the 56 MGD expansion is allowed.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NC0047384 effective July 1, 2014, and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in
Permit No. NC0047384.
The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approved by the Division.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by
the Division, such as the construction of additional or replacement wastewater treatment or disposal
facilities.
The Winston-Salem Regional Office, telephone number (336) 775-9800, shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the normal
office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed
in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and
specifications. Mail the Certification to: NPDES Unit, DWR/DENR, 1617 Mail Service Center,
Raleigh, NC 27699-1617.
Upon classification of the facility by the Certification Commission, the Permittee shall employ
a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater
treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or

I, , as a duly registered Professional Engineer in
the State of North Carolina, having been authorized to observe (periodically/weekly/full time)
the construction of the modifications and improvements to the T. Z. Osborne Wastewater
Reclamation Facility, located in Guildford County, hereby state that, to the best of my abilities,
due care and diligence was used in the observation of the following construction:
Demolition of existing traveling bridge tertiary filter components and retrofit of six (6)
new diamond cloth filter systems into the existing filter concrete structures, each system
filter area is 2,560 ft2 nominal 10 µm and is to include backwash pump and other filter
support features, specified filter system spare parts, piping, and controls; installation of
