Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16&SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter

➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
❑ Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section 1.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant
is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section 1.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated

cleanout as a means of correcting defects. If the defect is determined by the town to be in the portion
of sewer line lying between the main and the street right-of-way, or that portion which is installed by
the town as a part of the tap installation and that the installation of a cleanout will not resolve or has not
resolved the defect, then the town shall take the steps necessary to correct the defect including
replacement of a portion or all of the line as necessary between the main and the street right-of-way
boundary to ensure as standard as possible an installation with the fewest number of angled turns in the
line.
(b) Employees of the town shall not make repairs on private property or place sewer solvent in
commodes, vents, and the like, but, if needed, it may be put into the line at the point at which the
cleanout is being installed. Town employees shall not give sewer solvent to any persons whomsoever
for use inside residences or other structures.
§17-122 OIL AND GREASE CONTROL PROGRAM.
(a) Generally.
(1) No person shall discharge or cause to be discharged any of the following described water
or wastes either directly or indirectly into the sewerage facilities of the town:
(i) Any wastewater containing more than 100 parts per million by weight of total fats,
wax, grease, or oil, whether emulsified or not, or containing substances which may solidify or become
viscous at a temperature between 32EF and 160EF (OEC and 71EC). The use of chemical or biological
agents, physical methods, or any other means to dissolve, liquefy, suspend, disperse, emulsify, entrain,
or otherwise cause any oil, grease, or other similar material to flow through the sewer collection system
is prohibited;
(ii) Any substance which will cause interference, i.e., contribute to the treatment plant
violating its NPDES permit or the water quality standards of the receiving stream or any other federal
or state permit regulating the operation of the treatment plant;
(iii) Any materials which form excessive amounts of scum or foam which may interfere
with the operation of the sewerage facilities of the town or cause undue additional labor or expense in
the operation of same; and
(iv) Any trucked or hauled pollutants or wastewater except as specifically authorized by
the town.
(2) If the town determines to its satisfaction, that any prohibited discharge has taken place,
the town shall employ whatever enforcement actions it deems necessary.
(b) Requirements for oil and grease traps or interceptors.
(1) Town approved grease, oil, and sand traps or interceptors shall be provided for food
service establishments or operations, vehicular service facilities, and car washes when, in the opinion
of the town, they are necessary for the proper handling and control of wastewater being discharged to

public sewers containing grease, oil, or sand in excessive amounts. The traps or interceptors shall not
be required for private living quarters or dwelling units, but may be required for industrial or
commercial establishments, public eating places, hospitals, hotels, abattoirs, or other institutions. The
traps or interceptors shall be readily accessible for cleaning and inspection and shall be maintained by
the owner at his or her expense and in continuous efficient operation at all times. Whenever town or
sub-town inspection of the existing traps or interceptors results in a written notice for action on the part
of the person responsible for the trap or interceptor, the action shall be completed within the
compliance period granted by the inspecting authority.
(2) When retained or trapped material (including, but not limited to, oil, grease, sand, grit,
and the like) must be removed from grease, oil, and sand traps or interceptors, the material shall be
removed by pumping or other physical means and shall be hauled away for disposal in accordance with
applicable federal, state, and local regulations. No retained or trapped material in any form shall be
allowed to pass from the trap or interceptor into the sewer collection system. The use of chemical or
biological agents, physical methods, or any other means to dissolve, liquefy, suspend, disperse,
emulsify, entrain, or otherwise cause retained or trapped material to flow from the trap or interceptor
into the sewer collection system is prohibited. The owner shall provide the town, upon request, with
accurate information as to the ultimate disposal method and location of the material pumped or
otherwise removed from the trap or interceptor. All records shall be maintained for a minimum of
three years.
(c) Acceptance of hauled waste containing oil and grease.
(1) The town is to accept pumping originating from within the town from licensed septic
cleaning operators. The town shall keep records concerning each tank load to indicate origin,type, and
volume of waste.
(2) The schedule for accepting pumping shall be as published and distributed by the town to
those haulers licensed to discharge to the town.
(3) Pumping will be accepted only from:
(i) Wastewater of domestic origin from human sources (those generated from normal
human activities) from septic tanks; and
(ii) Grease traps containing oil and/or grease of animal and vegetable origin from
restaurants, hotels, or any other source, which prepares food for retail sales for direct human
consumption on the premises.
(d) Enforcement authority. If a user fails to comply with any provision of these rules and
regulations or fails to comply with any requirement of the town's permit system, including submission
of monitoring reports, or fails to produce an effluent that meets the applicable discharge limitations,
whether or not the user is a "significant" discharger, the town will take enforcement action pursuant to
the guidelines established in the Madison Sanitary Sewer Town Enforcement Response Guide. The
enforcement response guide is a document developed by the town, based on federal and state
regulations and guidance that provides town personnel with guidelines for appropriate enforcement

responses for various types and degrees of violations. The remedies provided in this section are not
exclusive. The town shall have the right, at its discretion, to take any, all, or any combination of these
actions against any noncompliant user. It is the town's intent that enforcement for violations will be
pursuant to the Madison Sanitary Sewer Town Enforcement Response Guide. However, the town
reserves the right to take any other action, either more or less stringent, that the town determines to be
appropriate against any user for any violation of any provision of the town rules and regulations,
wastewater permit or authorization, or orders issued. Further,the town is empowered to take more than
one enforcement action against any noncompliant user.
(e) Penalties for noncompliance.
(1) Any person violating any provision of these rules and regulations shall be deemed guilty
of a misdemeanor and, upon conviction, shall be punished by a fine not exceeding $100 or imprisoned
for a period not exceeding 30 days. Violations of discharge into the town's sewerage system will be
counted as separate violations for each parameter whose discharge limits or permit condition is
exceeded. Upon written notice to a person committing the violation, each day after receipt of the
notice that the violation is continued shall constitute a separate offense.
(2) Any person violating any provisions of these rules and regulations shall be subject to a
civil penalty not to exceed $2,000 for each violation. For purposes of penalty assessment, each day on
which a given violation occurs or continues shall be deemed a separate and distinct offense. In the case
of discharge limits with required monitoring less frequent than daily, the civil penalty may be assessed
for each day during the period of violation. Civil penalties assessed under this section shall be in
addition to any criminal penalties or any other penalties and remedies. All penalties assessed under the
provisions of this section shall constitute a debt payable to the town and shall constitute a lien against
the property of the person against whom the penalties are assessed.
(3) The town must issue all civil penalties in the form of an administrative order that states, at
minimum: the amount of civil penalty; the violation(s) for which the civil penalty is being assessed;
and the compliance date upon which the civil penalty must be paid in full. On or before the
compliance date specified in the administrative order, the user against whom the penalty has been
assessed must either pay the penalty in full or make a written request for an informal conference with
the town to attempt to negotiate a settlement agreement.
(4) If the user against whom the penalty has been assessed fails to pay the penalty in full or to
make a written request for an informal conference to negotiate a settlement agreement on or before the
compliance date specified in the administrative order, the town will issue to the user a show cause
order specifying which violation(s) is (are) charged and requiring the user to appear at an adjudicatory
hearing and show cause why the civil penalty should not be imposed. If an informal conference is
scheduled and the user fails to appear at the informal conference at the time and date specified for the
conference, or if an informal conference is conducted and the user and the town cannot negotiate a
settlement agreement at the informal conference, the town will issue to the user a show cause order
specifying which violation(s) is(are) charged and requiring the user to appear at an adjudicatory
hearing and show cause why the civil penalty should not be imposed.
(5) An adjudicatory hearing on the show cause order will be held before the Commission or

its designee appointed to conduct a hearing. All appeals from the final decision of the Commission or
its designee under the provisions of this section shall be heard in the Court of Common Pleas in the
town. All appeals from the final decision of the Commission or its designee under the provisions of
this section must be filed within 30 calendar days of the final decision of the Commission or its
designee.
(6) The remedies provided in this section are not exclusive. The town reserves the right to
take any other action, either more or less stringent, that the town determines to be appropriate against
any user for any violation of any provision of the town rules and regulations, wastewater permit or
authorization, or orders issued.
(f) Provision for recover of costs. Any nondomestic user, whether or not classified as significant,
shall be billed by the town for all costs associated with an enforcement action or actions in which the
town identifies the user as being in violation of any discharge permit or discharge authorization limit,
or any other requirement of the town's sewer use rules and regulations. Costs may include, but are not
limited to: monitoring at the user's discharge point, in the town's collection system, or at the town's
facilities which the town deems necessary to investigate and/or identify the user as the source of a
pollutant; costs of town personnel time and materials used in conducting any monitoring or
investigation which the town deems necessary to the enforcement action, or in any remedial efforts
which the town deems necessary to mitigate or correct the impact on any town facilities or processes of
a user's violation(s); costs of outside laboratories or consultants utilized by the town in the
investigation or in any remedial efforts which the town deems necessary to mitigate or correct the
impact on any town facilities or processes of a users violation(s); any fines or penalties imposed on the
town as the result of violation of any NPDES or other permit condition, any state or federal regulation,
or any law; and any attorneys' fees incurred by the town in connection with a violation of the rules and
regulations.
§ 17-123 USE OF SEWER SYSTEM BY PRIVATE SEWAGE DISPOSAL FIRMS; PERMIT
TO CONTRACTORS; PROCEDURE; PENALTY.
(a) Use of public sewer system by private disposal firms prohibited; certain contractors exempt
with permit;procedure for obtaining. It shall be a policy of the town to deny permission to discharge
any sewerage generated outside the town limits into the sewer system. However, realizing that septic
tank systems are the primary means of sewage treatment in the surrounding areas, the Wastewater Plant
Director will grant permission for access to the sewer system at the wastewater plant when it has been
determined that there is not another disposal source available to the contractor without his or her
incurring unreasonably high costs. In order for a contractor to receive a permit to discharge at the
aforesaid location for the aforesaid purpose,he or she will be required to:
(1) Make application for the permit at least three days prior to the time he or she will need
use of the system. The application shall be obtained from the Wastewater Plant Director;
(2) State the capacity of his or her tank truck or trucks that is to be used in hauling sewage to
the wastewater plant; and
(3) Make payment at the rate of$15 per tank truck each time sewage is disposed of in the
Wastewater Collection and Treatment System 47
system. The fee is based upon the truck's capacity not exceeding 1,000 gallons of sewage to be
disposed of. If a larger truck is used, the charge will be prorated according to the capacity of the tank
truck exceeding 1,000 gallons.
(b) Town to furnish identification cards or stickers to be affixed to affected tank trucks. The town
will provide the applicant with a suitable identification card or sticker to be affixed to each tank truck
to be used. The stickers will enable town personnel to readily identify applicants who have obtained
the proper permit.
(c) Agreement with town. The applicant shall enter an agreement with the town entitled"Disposal
of Domestic Septic Tank Waste Agreement" and meet all the stipulations and conditions of the
agreement prior to any disposal activity.
(d) Town employee to be present when disposal occurs; location of disposal manhole; hours and
charges. The town will assign an employee of the wastewater plant to the discharge manhole each
morning by 8:00 a.m. to assist in the operation. The employee will be available for activity until 3:30
p.m. each afternoon. Should the applicant wish to have the usage period increased before and/or after
these hours, or have to use the sewage discharge point on holidays, an additional charge of$25 will be
made to the applicant to help cover the expenses incurred by the town in paying overtime wages to
employees of the town beyond normal work hours.
(e) Penalty for violation of section. Any person using a public sewage manhole without having
first complied with this section shall be guilty of a misdemeanor and in addition to any penalties
provided for herein, subject to fine and imprisonment as provided by law. In addition to the criminal
penalty specified herein, any person disposing of sewage in the public sewage system, without first
obtaining a proper permit, shall be subject to a fine of$100 per day,per disposal,per tank truck,the fee
to be in the nature of a civil penalty under the provisions of G.S. § 160A-175(c).
§ 17-124 PROHIBITED DISCHARGE STANDARDS.
(a) General prohibitions. No user shall contribute or cause to be contributed into the POTW,
directly or indirectly, any pollutant or wastewater which causes interference or pass-through. These
general prohibitions apply to all users of a POTW whether or not the user is a significant industrial user
or subject to any national, state, or local pretreatment standards or requirements.
(b) Specific prohibitions. No user shall contribute or cause to be contributed into the POTW the
following pollutants, substances, or wastewater:
(1) Pollutants which create a fire or explosive hazard in the POTW, including, but not limited
to, wastestreams with a closed cup flashpoint of less than 140°F (60°C) using the test methods
specified in 40 CFR. 261.21;
(2) Solid or viscous substances in amounts which will cause obstruction of the flow in the
POTW resulting in interference but in no case solids greater than one-half inch in any dimension
(including wood, glass, ashes, sand cinders, and non-shredded garbage, etc.);

7.0 CAPITAL IMPROVEMENT PLAN
Based on the findings from this study, a Capital Improvement Plan has been developed for the
Town to follow for the next 20 years. A map and breakdown of each cost estimate is provided in
Appendix E. The projects listed are prioritized based on several factors such as risk of failure,
current condition, and importance to the overall system operating efficiently. It is recommended
that the Town budget for and plan to complete these projects within the time frame suggested
to avoid any major failures within the system. The following table lists each project based on
priority, with a cost estimate and a recommended time frame of completion:

existing aged infrastructure that is leading to I/I and NOVs within the collection system. This
project will replace approximately 7,300 I.f. of existing aged 8-inch collection lines and
approximately 29 manholes with new 8-inch PVC/DIP pipe and precast concrete manholes. A
majority of these lines to be replaced consist of VCP lines with brick manholes and have been
in service past their expected useful life. The Town has received several NOVs in this section
of the sewer system due to sanitary sewer overflows as a result of I/I. This project would
eliminate I/I in the lines replaced and greatly reduce the risk of further NOVs in this area of
the collection system. The estimated cost of this project is $1,905,000 and the Town should
plan to complete this project in the next 5 years.
7.2 Island Drive Outfall Replacement
This project is located in the western part of the Town and will replace existing aged
infrastructure.This outfall consists of VCP collection lines that were installed in the early 1970s
and have been in service past their expected useful service life. A majority of this project area
is located within the floodplain and is subject to infiltration during heavy rainfall events. In
addition,the Town has noted problems in the past of customers in this area tying sump pumps
for basements to their sewer services which leads to extraneous flow during heavy rainfall.
The Town has taken measures to put a stop to this, but it continues to be a problem. While
there are no recommendations to control this, the Town should continue to explore options
to keep its customers from doing this in order to reduce extraneous flow. This project will
replace approximately 6,900 I.f. of existing, aged 8-inch gravity sewer lines and approximately
29 manholes with new PVC/DIP pie and precast concrete manholes. The manholes within the
floodplain should be replaced with new precast concrete watertight manholes to reduce the
risk of infiltration during flooding events. Per NCDEQ rules regarding watertight manholes, a
vented manhole will be required every 1,000 feet or every other manhole, whichever is
greater. This area of the system is a source of I/I and as a result is causing a large amount of
extraneous flow to be discharged to Pump Station #1 during heavy rain events. This project
will help eliminate the I/I and reduce the risk of sanitary sewer overflows within this basin.The
estimated cost of this project is$1,415,000 and the Town should plan to complete this project
in the next 5 years.

7.3 Big Beaver Island Creek Outfall South Replacement
This project is located in the north western part of the Town and will replace existing aged
infrastructure that is leading to I/I and NOVs within the collection system. A large portion of
the project area is located within a floodplain and does not contain watertight manholes. This
is likely leading to I/I during heavy rainfall events.This project will replace approximately 3,200
I.f. of an existing aged 10-inch outfall line and approximately 13 manholes. The manholes
located within the floodplain will be replaced with new watertight manholes to reduce the risk
of infiltration during flooding events. The section to be replaced mostly consists of VCP lines
with brick manholes that were installed in the late 1960s and have been in service past their
expected useful life. The Town has received several NOVs along this outfall due to sanitary
sewer overflows which are a result of I/I within this area of the collection system. The
southernmost sections of this line were replaced in the early 2000s.This project would
eliminate the I/I within the lines replaced and greatly reduce the risk of further NOVs in this
area of the collection system. Due to the long length of this outfall, it has been broken up into
two separate replacement projects — north and south. The estimated cost of this project is
$630,000 and the Town should plan to complete this project in the next 5 years.
7.4 Big Beaver Island Creek Outfall North Replacement
This project is located in the north western part of the Town and will replace existing aged
infrastructure that is leading to I/I that has resulted in NOVs. This project will replace
approximately 6,000 I.f. of an existing, aged 10-inch outfall line and approximately 20
manholes with new PVC/ DIP pipe and precast concrete manholes.This section to be replaced
consists of VCP lines with brick manholes. The Town has received several NOVs along this
outfall due to sanitary sewer overflows which are a result of I/I within this section of the
collection system. This project would eliminate the I/I within the lines replaced and greatly

7.5 Basin 2 Sanitary Sewer Improvements
This project is located in the southwestern part of the Town and will replace existing aged
infrastructure that is leading to I/I within the system. This project will replace Pump Station #2
and Pump Station#3 which were installed in the early 1970s.These pump stations both consist
of wet wells with above ground pumps that have are located in underground fiberglass dry
wells adjacent to the wet wells. These fiberglass enclosures have served their purpose well
and are in good condition, however, the tight space within each dry well makes maintenance
and repairs very difficult. This project includes the relocation of the pumps and piping to an
above ground fiberglass enclosure located on a concrete pad with an alternative option to
convert this pump station to a submersible pump station. This project will also include the
installation of new permanent standby generators at each site. In addition, this project
includes the replacement of approximately 3,250 I.f. of aged 8-inch VCP gravity sewer lines
and approximately 20 manholes with new PVC/DIP pipe with precast concrete manholes.
These lines have been in service past their expected useful service life and are a source of I/I
within the collection system. While the pumps at each station have the extra capacity to
handle the extraneous flow,the I/I is sending extraneous flow to Pump Station#1 that is more
than this pump station is able to handle. This project, along with the Island Drive Outfall
project recommended should allow the Town to eliminate extraneous flow to an amount that
Pump Station #1 is capable of handling. The estimated cost of this project is approximately
$2,020,000 and the Town should plan to complete this in the next 5 years.
7.6 Pump Station #7 Rehabilitation
This project is located in the northwestern part of the Town and will rehabilitate existing aged
infrastructure that has been in service past it's expected useful service life. Pump Station #7
serves the industry Unifi and sewer from this pump station is discharged directly into the Town
of Mayodan collection system. This project includes the replacement of the existing pumps,
replacement of electrical and SCADA components on a new sheltered rack, replacement of the
existing piping and valves, coating the existing wet well and making spot repairs and any
additional necessary site repairs. This will also include the installation of a new permanent
standby generator on site. In addition, this project will include the construction of an access
road and necessary modifications to allow for vehicle access to the site. The estimated cost of
this project is $625,000 and the Town should plan to complete this project in the next 6 — 10
years.

CONTINGENCY PLAN
PUMP FAILURE CONTINGENCY PLAN
In accordance with Condition 1-7 of the Collection System Permit, the following is a contingency
plan for pump failure at all Sanitary Lift Stations.
• First the Maintenance personnel shall determine the nature of the problem.
• Maintenance personnel shall then notify the Public Works Director. If necessary,
emergency equipment will be mobilized and set to insure that a sanitary sewer
overflow does not occur.
• After it is determined if it is a pump or controls failure, the pump and control
cabinet shall be locked and tagged out before repairs are made.
• Major work such as bearing, seal replacement, or motor pump repairs will be
completed by a contractor in a timely manner.
There are small amounts of spare parts kept at the Public Works shop.
The large pumps are kept at Lift station #1.

• Lime & Straw is located at the Public Works Department or can purchased at

PUMP FAILURE CONTINGENCY PLAN
SEAL FAILURE
When a seal failure occurs, the pump is pulled and repaired by a motor repair shop.
BEARING FAILURE
Bearingarepurchased locallyor can be obtained from pumpmanufactures within a
24-
hour period. Our bearings are monitored weekly to prevent unscheduled down time.
PUMP FAILURE
When a pump failure occurs and it is determined to be the pump in most cases it is
pulled by the Public Utilities employees and replaced with a spare pump which is in
stock or sent to a repair shop to be fixed if spare pump is not available.
POWER FAILURE
Backup generators are on site, if there is a generator failure-Gann Brothers are notified
to pump lift station. The Town of Madison is notified to use their pump truck until a gas
powered by-pass pump is installed. Town of Mayodan keeps a by-pass pump at Public
Utilities building. CAT Generators is notified to repair on-site generator.
