Dear Mr. Mendenhall,
On September 22, 2022, Gary Hudson of this office met with Melinda Ward and Mike
Vernon to perform a Compliance Evaluation Inspection at the above referenced
wastewater collection system. This type of inspection consists of two basic parts: an in -
office file review and an on -site inspection of the treatment facility. The attached EPA
inspection form details the areas that were evaluated during this inspection.
The inspection of the facility was satisfactory. If you have any questions regarding the

*The permit became effective 6/01/2017 and expires on 5/31/2025.
*A FOG flyer is mailed in utility bills twice per year.
*A newly revised Capital Improvement Plan was provided that extends to 2032.
*Mr. Vernon and his staff do a very good job with record keeping of all maintenance and inspection activities.
*The West Avenue high priority line, and the Bridge Street pump station were inspected, and no discrepancies were noted.
*Mr. Vernon provided a good Spill Response Action Plan.
*The wastewater collection system appeared to be very well operated and maintained. No discrepancies were noted during
this inspection.
