Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary)
Site inspection was conducted by Kevin Mitchell from the Asheville Regional Office. The site was
walked from the water intake downslope to the outfall. Two settling ponds are being utilized to manage
turbidity and sediment loss. The outfall consisted of a small pipe and a separate notch/flume that
discharges into the creek. Water exiting the pond is causing erosion and headcutting back into the
dam structure. This needs to be addressed as soon as possible to prevent further erosion and the
potential of dam failure. This was discussed with Mr. Michalic and he agreed to address this issue and
submit a plan to the Division of Water Resources for approval.
