Modifications to the subject Attachment are as follows: Addition of the Prestage Farms Truck Wash WWTF (Permit No. WQ0016053) in Sampson County as an approved residuals source, with an approved maximum annual dry tonnage of 200 dry tons per year. The modified Attachment A shall be effective from the date of certification until February 28, 2023 (i.e., expiration date of the current permit), shall replace Attachment A of Permit No. WQ0000455 issued March 15, 2018 that was previously certified on July 2, 2021, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above-referenced permit. Please replace Attachment A of Permit No. WQ0000455 that was certified on July 2, 2022 with the modified Attachment A. If any parts, requirements, or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714.
