Enclosed please find a copy of the Field Audit Inspection Inspection form from the inspection conducted on
January 30, 2009. The Field Audit Inspection Inspection was conducted by Joel S. Shields of the Fayetteville
Regional Office. The facility was found to be in Non -Compliance with permit WQ0003626.

Inspection Summary:
Met with Campbell Soup managers, engineers, and ORC to discuss upcoming renewal of their permit. In addition Jim
Barber, Tony, Honeycutt and I conducted a site visit on Campbell Soup's spray irrigation fields. Fields D & E had a large
amount of run off that ran into a man made ditch which diverted the spray residual water into a metal pipe that drained
into a ditch running beside Oxindine Church Road.
In addition field (I) had a large area of residual water that had ponded and run off the field and up to the Campbell Soup
property line.
The above is in violation of Permit #WQ0003626- Operation & Maintenance Section II (3) Inclement weather shall not
affect the spray irrigation of wastewater unless the ground is in a condition that will cause run off on fields
A,B,C,D,E,F,G,H,I, and J.
The condition of the ground was too wet for the above fields to have residuals applied without causing run off. In addition
the vegetation on these three fields was unacceptable.
