Initial Report via email October 26, 2014; 3:34 pm Location of the inadvertent return: Stream 1, Back Creek Type of material: Bentonite clay and water slurry - standard HDD drilling mud. Estimated size of inadvertent return (quantity and area): An area approximately 25 feet wide by 25 feet long was affected. The contractor estimated less than 100 - gallons of slurry were released. The inadvertent return is localized along the north east side of the existing gas line right -of -way just north of the construction corridor; approximately 10 -feet from the east bank within the stream substrate. A visible deposition of drilling return, less than one inch, was noted within the contained area. Description of environmentally - sensitive areas, and their location in relation to the inadvertent return: The inadvertent return occurred within Stream 1, Back Creek. The area is localized and other Waters of the U.S. appear not to be affected by the event. Back Creek Inadvertent Return, Final Report SWE Project No. 7235 -14 -039 PSNC Line T021 B, Stream 1 November 3, 2014 drains to the Haw River which is approximately 2,000 linear feet downstream from the site. Description of the methods used to clean up and secure the site: The contractor removed drilling mud that had accumulated within the coffer dam and disposed of the material by pumping the fluid, via submersible pump, back to the up gradient mud pit located within uplands. Upon completion of drilling activities, the inadvertent return location was evacuated of water and any deposited material removed by hand. Upon completion of clean -up activities, the coffer dam was removed and the stream allowed to flow in a normal fashion. Stream banks were not disturbed during clean -up activities. General comments: This Final Report is being provided as a follow -up to our initial report, dated October 26, 2014. Clean up activities initiated and were completed on October 29, 2014, please see attached Representative Photographs.
