This letter is a reminder that the above referenced permit is set to expire on 10/31/2023.Per 15A NCAC
02T.0109,a renewal application should be received 180 days prior to expiration,which in this case is no later
than 5/4/2023. If this date has passed,please respond immediately to avoid a compliance violation. The
application for the renewal can be downloaded using the following steps:
• Go to:https://deq.nc.gov/about/divisions/water-resources/water-resources-permits/wastewater-

• Click on Wastewater Collection System Application—New/Renewal(Form CSA 04/16)
The original application,signed by an authorized signing official,with requisite supporting documentation as
