This is a provisional general permit verification. Your proposed work may be authorized by the above referenced general
permit provided a 401 Certification is issued or waived. Once a 401 Certification is issued or waived, the Corps will
provide you with a final general permit verification. Your proposed work shall not commence until after the 401
Certification has been issued or waived AND until a final general permit verification has been provided to you. Additional
special conditions may be added to the final general permit verification, including the following:
1) 1) In order to compensate for impacts associated with this permit, mitigation shall be provided in accordance
with the provisions outlined on the most recent version of the attached Compensatory Mitigation Responsibility
Transfer Forms (2). The requirements of this form, including any special conditions listed on this form, are hereby
incorporated as special conditions of this permit authorization.
2) Temporary discharge of excavated or fill material into wetlands and streams will be for the absolute minimum
period of time necessary to accomplish the work, and such authorized crossings will be restored per the
following:
a. All authorized temporary wetland impacts must be returned to pre -disturbance grade and contour and
re -vegetated per "Line 142 Pipeline, E&SC Details, Clayton, North Carolina", Drawing Number PNG C-
142-0001085, Detail 15. In wetland areas where pipeline installation via trenching is authorized, wetland
topsoil must be segregated from the underlying subsoil, and the top 6 to 12 inches of the trench must be
backfilled with topsoil from the trench. Seeding of disturbed wetland areas must be done per "Line 142
Pipeline, E&SC Seeding Requirements, Clayton, North Carolina", Drawing Number PNG C-142-0001081,
"General Seeding Requirements for Wetland Areas."
b. All authorized temporary stream impacts must be returned to pre -disturbance grade and contour, and
stabilized per "Line 142 Pipeline, E&SC Details, Clayton, North Carolina", Drawing Number PNG C-142-
0001082, Details 2 and 5 (Stream Impacts 531 and 535). Seeding of stream banks in these areas must be
done per Drawing Number PNG C-142-0001081, "General Seeding Requirements for Riparian Areas."
3) Wetland crossings W2, W4C, W4D, W4A-A, W4E, W6A, and Stream crossings 57, 58, 59, 510, 511, 512, 518, and
the Neuse River must be accomplished through trenchless methods, such as HDD or similar. Note that no
discharges of dredged or fill material are authorized within the Neuse River whatsoever.
4) The permittee shall use a contracted environmental compliance officer(s) who will remain on -site throughout the
construction period. This individual must ensure compliance with all general and specific Department of the
Army and North Carolina Division of Water Resources permit conditions and certifications. Once construction
begins within crossings subject to this authorization, compliance reports must be submitted to the Corps of
Engineers, Raleigh Regulatory Field Office and North Carolina Division of Water Resources on a bi-weekly basis,
until construction within these areas is complete.
