Dear Mr. and Mrs. Cobb:
On August 31, 2022, staff of the North Carolina Division of Water Resources Winston-Salem
Regional Office (DWR) performed a compliance inspection of the subject wastewater irrigation
system. This compliance inspection was conducted by DWR staff person Jim Gonsiewski. The
inspection generally reflects compliance with the permit.
Please refer to the enclosed compliance inspection report for additional observations and comments.
Please note that this permit will expire on December 31, 2022. If you have any questions concerning

Inspection Summary:
On August 31, 2022, staff of the North Carolina Division of Water Resources Winston-Salem Regional Office (DWR)
performed a compliance inspection of the subject wastewater irrigation system. This compliance inspection was conducted
by DWR staff person Jim Gonsiewski. The system was in good working order and well maintained. The permit will expire on
12/31/2022. The inspection generally reflects compliance with the permit. .

Arrived on site at 11:05 AM, 08/31/2022. Met with the owners, Jeff and Connie Cobb.
Walked through the system. Everything was in good operational order. According to the
owners, they do not have any problems with the system. The alarm for the system works and
all the spray heads are clear of obstructions. The spray field has been mostly cleared. No
evidence of any ponding or runoff was observed in the field. The fence is in good condition with
no breaks noted. Left residence at 12:10 PM.
The permit for the system expires on 12/31/2022. There is one (1) permit fee invoice that
is overdue (2021). A copy of this invoice was sent to the owners.
