Division of Water Resources' Central and Regional staff has reviewed the application package
received June 30, 2022. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on October 26, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through E, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with an

A. High -Rate Infiltration System — Renewal (FORM: HRIS-R 02-21):
1. The Back-up ORC information in Section V of the application was not completed. In accordance
with 15A NCAC 08G .0201(1), owners of classified water pollution control systems shall designate
one or more Back-up ORC(s) who possesses a valid certificate of the type of system in addition to
the primary ORC. This section shall be updated in an updated application to provide the information
for a Back-up ORC who possesses a WW-3 or above certificate.
2. The open -atmosphere structure in Section VI and the monitoring wells in Section VIII do not
include these components' latitude and longitude coordinates. This information shall be provided
in an updated application to verify their locations.
Section VI — Open -Atmosphere Structures lists a 116,000 gallon metal lined treatment unit.
However, there is no 116,000 gallon unit listed in the existing permit facility description. Please
clarify which unit this is referring to.
4. The sites in Section IX — Infiltration Sites are both listed as Enviracon Utilities, Inc. This
information should correspond to the site IDs as they are listed in Attachment B. This section shall
be updated with the correct site ID nomenclature to verify their information.
B. Attachment D — Certificate of Public Convenience and Necessity:
1. You noted in the HRIS 02-21 that the Applicant is a Privately -Owned Public Utility, but the
Certificate of Public Convenience and Necessity was not provided pursuant to 15A NCAC 02T
.0115(a)(1). Please provide this documentation to demonstrate this the applicant is designated as a
public utility by the NC Utilities Commission authorized to provide service to the specific project
area.
C. Attachment G — Easement, Encroachment, and Lease Agreements:
1. It is indicated in this section that the Permittee owns all land associated with the system, but the
Deeded Owners listed in Section IX is the Island Beach and Racquet Club Condominium Owners
Association, Inc. A copy of all easements, lease agreements, and encroachment agreements
allowing the Permittee to operate and maintain the system on property not owned by the Permittee
shall be provided in accordance with 15A NCAC 02T .0116(c).
D. Attachment I — Compliance Schedules:
Condition I.3. of the previous permit required that MW-4 be permanently abandoned within 60
days of the effective date of the permit, and a Well Abandonment Record (FORM: GW-30) should
have been provided within 30 days of abandonment. The GW-30 for MW-4 shall be provided.

E. Attachment L — Setback Waivers:
1. It was noted in this Attachment that the existing permit includes setback waivers. However, no
setback waivers were provided that were in accordance with 15A NCAC 02T .0706(d). Any
existing setback waivers for this facility shall be provided to be included in the permit.
