Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of dual culverts on SR 1284 in
Northampton County. WBS # 1.206611
The North Carolina Department of Transportation is requesting the issuance of a Nationwide
Permit 3 and corresponding 401 Water Quality Certification for the replacement of dual culverts on SR
1284 in Northampton County. The culverts are located on SR 1284/ Butler Drive approximately 0.35 mile
east of the intersection with SR 1230. The culverts are located in an unnamed tributary to Lake Gaston,
which is classified by NCDWR as class "WS- IV,B;CA" waters. The existing culverts are (1) 30" diameter
X 56' long corrugated metal pipe and (1) 24" diameter X 50' long reinforced concrete pipe. These culverts
will be removed using NCDOT's guidelines for culvert removal. The proposed corrugated aluminum pipe
will be placed on the existing alignment as shown in the permit drawings. The new culvert is 96" X 60'
with a 1 foot sill plate attached to the inlet. The new culvert will be placed 1 foot below the surveyed
stream bed. Jurisdictional wetland and surface water impacts have been minimized on this project, resulting
in 4 linear feet of stream impact and no wetland impacts.

Project area consist of NCDOT Northampton County SR 1284 with intermittent stream passing under the roadway
through (1) 30 "X56' CMP and (1) 24 "X50' RCP. The adjacent property use is a mix of residential and forested area.

3d. Explain the purpose of the proposed project:
Replace the deteriorating culverts, improve the stream conveyance, and maintain a stabile road way along SR 1284 for
the safety of the traveling public.
3e. Describe the overall project in detail, including the type of equipment to be used:
The existing (1) 30 "X56' CMP and (1) 24 "X50' RCP will be entirely removed using NCDOT guidelines for pipe removal. A new
CAP measuring 96 "X60' will be placed as shown in the provided permit drawings. The new pipe will be buried 1 foot below the
surveyed stream bed, and include a 1' sill plate on the inlet. Once the pipe is in place and back filled, a new roadway surface
and grassed shoulder will be established. The banks will be stabilized following NCDOT erosion control bmp's, and include rip
rap stabilization. Dewatering operation will be in place to prevent any sediment loss. The project will result in 4 linear feet of
stream impacts and no wetland impacts. This work will be accomplished using a track excavator, backhoe, and dump trucks.
