(NOTE: Permitting of future lines, internal to the future cabin site, will be addressed with a future submittal; although, the flow request for the cabins is INCLUDED within this submittal). This project is located in the planning jurisdiction of the Town of Smithfield, in Johnston County. Construction includes an 8-inch "gravity" Town sewer line extending from the County's existing 12-inch gravity line, to serve the future units proposed on site, which requires approval by NCDEQ. Because the small gravity extension will be a Town -owned sewer, the Town of Smithfield is indicated as "Applicant" for the enclosed application form. The proposed sewer facilities will serve 14 future units, which generates the additional flow requested within this application. We are hereby applying for a "Fast -Track" review process; with a total flow request of 2,800GPD (or 0.0028 MGD). Enclosed herewith are the following documents for your review: 1) Fast Track Sewer System Extension Application, Form FTA 06-21--1 original + 1 copy signed by myself as Engineer, and signed by Applicant (Town of Smithfield). 2) Check in the amount of $480 for the required Fast -Track Application review fee 3) Flow Tracking/Acceptance Forms, Form FTSE 10-18 —1 signed original + 1 copy for each of the downstream owners/operators (from project to treatment plant) as follows... • Johnston County —Downstream Collection System & Wastewater Treatment Facility 4) Site Maps —1 original + i copy of each of the following... • 8.5" x 11" Color Copy of USGS Topographic Map, showing entire project area and adjacent surface waters • 8.5" x 11" Street Level, Aerial Map, showing general project area Please Note: This same submittal was transmitted to your office back in April of this year, but was subsequently retracted by the applicant just after submittal, and prior to being processed by your office.
