To Whom It May Concern::
On behalf of Ms. Christine England, DCA Lexington Properties, LLC, we are pleased to submit
the enclosed information for your review and approval of the stormwater management permit for
the subject project.
We are enclosing the following
I . Two complete sets of construction drawings (folded to 8.5" x 14"), as issued, "Issued For
Stormwater Review" dated, August 12, 2022;
2. one original and one copy of the Stormwater Management Permit Application Form;
3. one original of the Supplement-EZ Cover Page;
4. one original of the signed and notarized O&M Agreement (high density) and required
supplement form;
5. a "Stormwater Narrative and Calculations" describing the stormwater management for
this site;
6. a USGS map identifying the site location;
7. a check in the amount of $505.00, as payment of the application processing fee;
8. a copy of the most current deed for the subject property (Deed Book 2472, Page 0112), as
shown on the enclosed Plat of Survey (Plat Book 69, Page 2), recorded in Davidson
County Register of Deeds;
9. a copy of the Articles of Organization on DCA Lexington Properties, LLC, the applicant;
10. a copy of the Articles of Incorporation on Davidson Charter Academy, Inc.;
11. a copy of the Board Meeting Minutes for Davidson Charter Academy, Inc.;
12. and, a copy of the By -Laws for Davidson Charter Academy, Inc. -Signed.
