Office EEl\BED JUL 0 1 2016 DWR SECTION INFORMATION PROCESSING UNIT Altamont Environmental, Inc. (Altamont) performed a groundwater sampling event at the subject facility (the Site) on May 4, 2016. A Site location map is included as Figure 1. Altamont performed the groundwater sampling event in accordance with the North Carolina Department of Environmental Quality (DEQ) wastewater- irrigation system permit number WQ0004797 (reissued September 3, 2015). Altamont collected - groundwater samples from six monitoring wells: MW-4, MW-5, MW-6, MW-713; MW-8, and MW-9. Monitoring well MW-9 was installed on April 14, 2015 to replace the previously damaged, monitoring well MW-2R. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services, Inc. (Pace), a .North Carolina -certified laboratory, for analysis of the parameters specified in the permit. Table:1 summarizes the current and historical laboratory analytical results for microbiology and wet chemistry. All parameters analyzed, except for total organic carbon .(TOC) and nitrate, were below the permit limits and the Title 15A, North Carolina Administrative Code (NCAC), Subchapter 2L.0202 standard (21- standard). TOC detections are summarized as follows: The permit limit for TOC is 10 milligrams per liter (mg/L) • As with previous sampling events, the TOC concentration in the sample collected from MW-8 exceeded the permit limit. TOC also exceeded the permit limit in samples collected from MW-7R and MW-9 (see Tabled). • TOC concentrations in samples MW-4, MW-5, and MW-6 collected during this event (May 2016) were below the permit limit (see Table 1). Nitrate detections are summarized as follows: • The permit. limit for nitrate is 10 mg/L. • Nitrate concentrations exceeded the permit limit in samples. collected from MW-7R and MW-8 during this event (see Table 1). • . Nitrate concentrations in samples MW-4, MW-5, MW-6, and MW-9 collected during this event were below the permit limit (see Table 1). P:\Clement Pappas\Reports\2016 2081.01\May\Cover Letter —May 2016.Doc Blake Kehoe June 29, 2016 Page 2 of 4 Upon notification of the nitrate exceeda.nces. in MW-7R and MW-8, Clement Pappas requested that Altamont' re -sample these monitoring wells for nitrate. In order to further investigate the potential source of nitrate, Clement Pappas also requested that Altamont collect surface -water samples (samples titled "Upstream" and "Downstream") from Mud Creek, which is located along the southern edge of the property boundary and . flows parallel to the property line in an east -to -west -direction. The Upstream sample represents surface -water quality upstream of the irrigation system, and the Downstream sample represents surface -water quality downstream of the irrigation system. Surface -water sample locations are indicated on Figure 2. Altamont mobilized to the Site on June 9, 2016 to collect samples for nitrate analysis at MW-713 and MW-8, as well as from the Upstream and Downstream surface sampling locations. Two samples were collected at each location and submitted to separate analytical laboratories as a quality control measure. One set of samples was sent to Pace foranalysis, and a second set of samples was sent to Prism Laboratories, Inc. (Prism). The Pace re -sample results are summarized as follows: • Nitrate exceeded the permit limit in samples collected from MW-713 and MW-8 at similar concentrations as the first round of sampling (see Table 1). The Upstream and Downstream surface -water samples had nitrate concentrations of 0.85 mg/L and 0.86 mg/L, respectively. The Prism re -sample results are summarized as follows: • Nitrate exceeded the permit limit in samples collected from MW-713 and MW-8 at similar concentrations as the first round of sampling (see Table 1). The Prism analytical results were consistent with the results reported by Pace for the re -sample event. • The Upstream and Downstream surface -water samples analyzed by Prism had nitrate concentrations of 0.74 mg/L and 0.78 mg/L, respectively. Following the receipt of the analytical results from the June 9, 2016 nitrate sampling event, Altamont began reviewing and analyzing data provided by Clement Pappas that included analytical data for wastewater effluent, temporal spray irrigation records, and other pertinent data: Upon review of this data, the following conclusions were made: • Clement Pappas analyzes wastewater effluent for three types of nitrogen -containing parameters, including Total Kjehldahl Nitrogen (TKN), ammonia, and nitrate/nitrite. Of these parameters, TKN is consistently measured in significant concentrations, with ammonia and nitrate/nitrite generally measured at substantially lower concentrations. • Monthly TKN concentrations measured in wastewater effluent were relatively consistent from June 2014 through May 2016 with.no discernable outliers. Based on this review and analysis of effluent and irrigation data; Clement Pappas calculated a rolling 12-month TKN-application rate for its 13 permitted spray irrigation zones. This rolling 12-month application rate began in June 2015,and ended in May 2016. Rolling 12-month TKN application rates are as.follows: . • Zone 1-294_pounds Zone 2-115 pounds • . Zone 3=278 pounds • Zone 4=380 pounds P:\Clement Pappas\Reports\2016 2081.01\May\Cover Letter_May 2016.Doc Blake Kehoe June 29, 2016 Page 3 of 4 • Zone 5A-393 pounds • Zone 513-156 pounds • Zone 6-681 pounds • Zone 7A-540 pounds • Zone 713-199 pounds Zone 8-896 pounds • Zone 9-1,070 pounds • Zone 10-348 pounds • Zone 11-379 pounds If an assumption was made that TKN was influencing nitrate concentrations in groundwater, these application rates would suggest that the top four highest nitrate concentrations would be -measured in monitoring wells MW-7R, MW-9,, MW-4, and MW-8, respectively. This expectation would be based on the spray irrigation zones upgradient of these monitoring wells having received 12-month rolling TKN application rates of approximately 2,122 pounds (Zones 5B, 8, and 9), 1,074 pounds (Zones 5A and 6), 1,067 pounds (Zones 1, 2, 3, and 4), and 727 pounds (Zones 10 and 11), respectively. Instead, the following is the case: • MW-8—highest nitrate concentration of 26.3 mg/L (727 pounds TKN applied upgradient) • MW-713—second highest nitrate concentration of 11.2 mg/L (2,122 pounds TKN applied upgradient) • MW-9—nitrate concentration less than 1 mg/L (1,074 pounds TKN applied upgradient) • MW-4^nitrate below reporting limit of 0.020 mg/L (1,067 pounds TKN applied upgradient) Therefore, it would appear that the TKN concentrations in the wastewater effluent that is applied to the spray irrigation zones are not the source of the nitrate exceedances in monitoring wells MW-7R and MW-8. Table 2 summarizes current and historical groundwater elevations. With the exception of monitoring well MW-9, all monitoring well concrete pads and top -of -well casing elevations were surveyed by Wes Cole Land Surveying, PA on November 21, 2012. Monitoring well MW-9 was surveyed by Wes Cole Land Surveying, PA on May 29, 2015. The -surveyed elevations are indicated on Table 2 and Figure 2. In accordance with the spray irrigation permit, Clement Pappas is required to submit the following by June 30, 2016 (on or before the last working day of the month following the sampling month) to the DEQ: Laboratory analytical reports and chain -of -custody forms (Attachment A) • The well sampling logs and equipment documentation and instrument calibration data sheet (Attachment B) • DEQ GW-59A.—Compliance Report Form (Attachment C) • DEQ GW-59—Groundwater Quality.Monitoring Compliance Report Form for each monitoring well (Attachment D) Based on Altamont's review of available data from wastewater effluent and TKN application rates to the spray irrigation zones, it would not appear that the nitrate exceedances in monitoring wells MW-7R and MW-8 can be attributed to the spray irrigation system. -Altamont proposes to complete the next scheduled groundwater sampling event in November 2016..
