(Altamont) performed a groundwater sampling event at your facility on November 4, 2015. A site location map is included as Figure 1. Altamont performed the groundwater - sampling event in accordance with the North Carolina Department of Environmental Quality (DEQ), formerly known as the Department of Environment and, -Natural Resources (DENR), spray irrigation system permit number WQ0004797 (permit most recently reissued on September 3, 2015). Altamont collected groundwater samples from six. monitoring wells: MW-4, MW-5, MW-6, MW-7R, MW-8, and MW-9. Monitoring well MW-9 was installed on April 14, 2015 to replace the previously damaged monitoring well MW-2R. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services, Inc. (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1 summarizes the current and historical laboratory analytical results for microbiology and wet chemistry. All parameters analyzed, except total organic carbon (TOC) and fecal coliform, were below the permit limits and North Carolina Administrative Code, Title 15A, Subchapter 2L.0202 Standards (21L standards). The TOC concentrations can be summarized as follows: • The permit limit for TOC is 10 milligrams per liter. • -Similar to previous sampling events, the TOC concentration in the MW-8 sample collected during this event (November 2015) was in excess of the permit limit. • ,TOC concentrations in samples MW-4, MW-5, MW-6, MW-7R, and MW-9 collected during this event (November 2015) were below the permit limit. Fecal coliform concentrations can be summarized as follows: • The 2L standard for total coliform is 1 colony -forming unit (CFU) per 100 milligrams. There is not a specific permit limit or 2L standard for fecal coliform; however, total coliform includes fecal coliform. Fecal coliform concentrations exceeded the 2L standard for total coliform in monitoring wells MW-5, MW-6, and MW-8. • Fecal coliform was not detected in monitoring wells MW-4, MW-7R, and MW-9. In accordance.with permit requirements samples are analyzed for VOCs once annually, during the month of November.. All VOCs analyzed, except tetrachloroethene, were below permit limits and 2L standards. VOC concentrations can be summarized as follows: • Three VOCs (tetrachloroethene, trichloroethene, and cis-1, 2-dichloroethene) were detected in . monitoring well MW-5. Of these VOCs, only tetrachloroethene was detected above its 2L standard of 0.7 micrograms per liter (pg/L). • Toluene was detected for the first time in monitoring well MW-4 at a concentration of 0.75 pg/L. This detection is well below the 2L standard of 600 pg/L. Table 3 summarizes the current and historical groundwater elevations. With the exception of monitoring well MW-9, all of the'monitoring well concrete pads and top -of -well casing elevations were surveyed by Wes Cole Land Surveying, PA on November 21, 2012. Monitoring well MW-9 was surveyed by Wes Cole Land Surveying, PA on May 29, 2015. The surveyed elevations are indicated on Table 3 and on Figure 2. In accordance with the spray irrigation permit, Clement Pappas is 'required to submit the following by December 31, 2015 (on or before the last working day of the month following the sampling month) to the DEQ: • Laboratory analytical report and chain -of -custody form (Attachment A); the well sampling logs and equipment documentation and instrument calibration data sheet is also included (Attachment B) • DEQ GW-59A—Compliance Report Form (Attachment C) • DEQ GW-59—Groundwater Quality Monitoring Compliance Report Form. for each monitoring well (Attachment D) Altamont proposes to complete the next scheduled groundwater sampling event in May 2016.
