(Clef 125 Industrial Park Road Hendersohville,.North Carolina Dear Mr. Kehoe: J . . ; Re9\0aO0eta0\os]i r'' i ''iNRSECTION vent. UK Altamont Environmental, Inc. (Altamont) performed a, groundwater sampling event at your facility on May 13,- 2015. A site location map is included as Figure 1. Altamont performed the groundwater sampling event in accordance with the North Carolina Department of Environment and Natural Resources (DENR) spray. irrigation system permit number WQQ004797 (permit issued December 29, 2006 and reissued on October 12, 2012). Altamont collected groundwater samples from six monitoring wells: MW-4, MW-5, MW-6, MW-7.R, MW-8, and newly installed MW-9. Monitoring well MW-9 was installed on April 14, 2015 to replace the previously damaged monitoring well MW-2R. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services, Inc. (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1 summarizes the current -and historical laboratory analytical results for microbiology and wet chemistry. All parameters analyzed, except total organic carbon (TOC), were below the permit limits and North Carolina Administrative Code, Title 15A, Subchapter 2L.0202 Standards,(2L Standards). The TOC concentrations can be summarized as follows: • The permit limit for TOC is 10 milligrams per liter. • Similar to previous sampling events, the TOC concentration in the MW-5 and MW-8 samples collected during this event (May 2015) was in excess of the permit limit. • TOC concentrations in samples MW-4, MW-6, MW-7R, and MW=8 collected during this event (May .2015) have decreased since the previous sampling event (November 2014). Table 2 summarizes the current and historical groundwater elevations. With the exception of monitoring well MW-9, all of the monitoring well concrete pads and top of the well casings elevations.were surveyed by Wes Cole Land Surveying, PA on November 21, 2012. Monitoring well MW-9 was surveyed by Wes Cole Land Surveying, PA on. May 29, 2015. The surveyed elevations are indicated on Table 2 and on Figure 2. In accordance with the spray irrigation permit, Clement Pappas is required to submit by June 30, 2015 (on or before the last working day.of the month following the sampling month) to the DENR the following: • Laboratory analytical, report and chain -of -custody form (Attachment A); the well sampling logs and equipment documentation and instrument calibration data sheet is also included (Attachment B) • DENR GW-59A—Compliance Report Form (Attachment C) P:\Clement Pappas\Reports\2015\May\Cover Letter -May 2015.Doc Mr. Blake Kehoe June 10, 2015 Page 2 of 2 • DENR GW-59—Groundwater Quality Monitoring Compliance, Report Form for each monitoring well (Attachment'D) Altamont proposes to complete the next.scheduled groundwater sampling event in .November-2015. The spray irrigation permit expires on November 30, 2015. A renewal request was submitted May 29, 2015. ND - Not Detected at or above adjusted reporting limit. J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit. MDL -Adjusted Method Detection Limit. PQL - Practical Quantitation Limit. RL - Reporting Limit. S -Surrogate 1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is a combined concentration. Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery, and RPD values. X ' If the answer to question 5 is "NO", skip to section S. If the answer to question 5 is "YES", list in the space provided below, each well with constituent(s) exceeding standards, concentrations) reported, and sample collection date for each occurrence (for the last two years). 5/13/15: MW-5 = 21.0 mg/L, MW-8 - 19:7 mg/L - 11/18/14: MW-4 - 28.7 mg/L, MW-5- 16.0 mg/L, MW-6 - 16.3 mg/L, MW-71i = 19.1 mg/L, MW-8 17.6 mg/L, 5/22/14: MW-9 = 17.6 mg/L 11%20/13: MW-B - 21.0 mg/L 5/16/13: MW-2R - 13.5, MW-6 = 31.8, MW-7R - 28.0 mg/L, MW-8 15.8 mg/L 6 Are the monitoring wells listed in section 5 located at or beyond the review boundary? YES NO MN-5 and MW-8 are beyond the review boundary •X If the answer is "YES", a groundwater quality problem maybe occurring. CONTACT THE REGIONAL OFFICE IMMEDIATELY FOR GUIDANCE. If the answer is "NO" monitoring wells may be improperly located; contact the Regional Office. 7 is the permittee implementing previously approved actions required by the Division -.involving. this YES NO groundwater quality problem? X If the answer to question 7 is 'YES", describe those. actions in the space provided below. If the answer to question 7 is "NO" contact the Regional Office within 90 days: an evaluation may be ' required to determine the impact the waste disposal system is having at the review and compliance boundaries surrounding this facility. Failure to do so may subiect the permittee to a Notice of Violation. fines, and/or penalties. Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCS and is possibly naturally occurring. g .The person completing this portion (GW-59A) of the monitoring report should sign below and submit this . form with GW-59 forms forrequired wells to the address provided at the top of the current GW-59 form. ll;hereby:ac'knowledgetnat the above inforrnation was evaluated and the Information submitted in this Ireport,(Comph__iice Report• GW-59A) is true and complete to the best of my knowledge i- Signature of Permittee (orAuthorized Agent) Dale , 0V-59A 12/811003 bUbIVII I t-UKIVI UN Yt-LL.UVV NANtK UNLY DEPARTMENT QFENViRONMENT &NiRTURAL RESOURCES GROUNDWATER QUALITY MONITORING: DIVISION OF WATER RESOURCES , INFORMATION PROCESSING UNIT COMPLIANCE REPORT FORM 1517 MAIL SVRVtcs CENTER, RALEIGH, NC27699-1617 'Phone: 919-807-63 1 0e FACILITY INFORMATION Please Print clearly or Type PERMIT Number: Expiration Date: 11 / 3 0 / 2 01.5 Facility Name: Clement Pappas Non -Discharge WQ0004797 UIC Permit Name (if different): NPDES Other Facility Address: 125 Industrial Park TYPE OF PERMITTED OPERATION BEING MONITORED Hendersonville (street) NC 28792 CountyHenderson ❑Lagoon ❑Remediation: Infiltration Gallery (Cthr) (State) (Zip) N Spray Field ❑ Remediation: Contact Person: Mr. Blake Kehoe Telephone#: 828 - 6 93 - 0711 ❑ Rotary Distributor ❑ Land Application of Sludge Well Location/Site Name: Irrigation Area No. of wells to be sampled: 6 ❑ Water Source Heat Pump ❑ Other: WELL ID NUMBER (from Permit): M_ 9 Date sample collected: 5 / 1.3 / 2 015 FIELD ANALYSES: Well Depth: 18.1 ft.
