(Altamont) performed a groundwater sampling event at your facility. on November 18, 2014. A site location map is included as Figure 1. Altamont performed the groundwater sampling event in accordance With the North Carolina Department of Environment and Natural -Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006 and. reissued on October 1.2,.2012). Altamont collected groundwater samples from five monitoring wells: MW-4, MW-5, MW-6, MW-7R, and MIN-8. A sample could not be collected from MW-2R during this sampling event. The casing appears to be bent and the disposable bailer could not be lowered into the water column. The monitoring well will be evaluated prior to the next sampling event. The locations of the monitoring wells are.. shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace -Analytical Services, Inc. (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1.summarizes the current and historical laboratory analytical.results for microbiology and wet chemistry. All parameters analyzed, except fecal coliform and -total organic carbon (TOC), were below the permit limits and North Caro lina;Administrative Code,_Title 15A, Subchapter 2L.0202 Standards (2L standards). Fecal coliform concentrations can be summarized as follows: The permit limit- for fecal: coliform is 1.0 colonyforming units (CFU) per 100 milliliters (mQ. • The.fecal-coliform concentration in MW-5 was above the laboratory reporting limit during this sampling event (November 2014) and represents an increase from the previous sampling event (May 2014), but the laboratory reporting limit was elevated above the permit limit in the previous event. • The fecal coliform concentration in the MW-6 sample collected during this event (November 2014)_- was above the permit limit and represents an increase from the previous sampling event (May - 2014). • . As in the May 2014 sampling event, the fecal coliform .concentrations in MW-4; MW-7R, and MW-8 remain below the laboratory reporting limit in the samples collected during this event (November 2014). • The laboratory reporting limit for sample MW77R (20 CFU per 100 mL) was elevated and above the - permit limit. The TOC concentrations can be. summarized as follows: • The permit limit for TOC is 10 milligrams per liter. Similar to previous sampling events, the TOC concentration in the MW-4, MW-5, MW-6, MW-7R, and MW-8 samples collected during this event (November 2014) was in excess of the permit limit. • TOC concentrations in samples MW-4, MW-5, MW-6, MW-7R, and MW-8 collected during this event (November 2014) have increased.since the.previous sampling event (May 2014). Table 2 summarizes the current and historic grouridwater,laboratory analytical results for volatile organic compounds (VOCs). Concentrations of tetrachloroethene and trichloroethene were detected in excess of the 2L standard in MW-5. The tetrachloroethene, trichloroethene, and cis-1,2-dichloroethene concentrations have decreased since the previous sampling event in November 20.13. All other parameters analyzed were.' below 2L standards. Table 3 summarizes the current and historical groundwater elevations. All of the monitoring well concrete plads and top, -of -the -well -casing elevations were resurveyed by Wes Cole Land Surveying, PA. on November .21, 20.12. The elevations are reflected on Table 3 and on Figure 2. In accordance with the spray irrigation permit, Clement Pappas is required to submit by December 31, 2014 (on or before the last working day of the month following the sampling month) to DENR the following: • Laboratory analytical report and chain -of -custody form (Attachment A); the well sampling logs and equipment documentation and instrument calibration data sheet is also included (Attachment B) • DENR GW-59A—Compliance Report Form (Attachment C) • DENR GW-59—Groundwater Quality Monitoring Compliance Report Form,for each monitoring well (Attachment D) Altamont proposes to complete the next scheduledgroundwater sampling event in May 2015.
