(Altamont) conducted a groundwater sampling event at your facility on May 22, 2014. A site location map.is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with the North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006 and reissued on October 12, 2012). Altamont collected groundwater samples from six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7R, and MW-8. Monitoring well MW-7R was installed on June 28, 2012 to replace MW-7, which was destroyed in June 2012 by field equipment. Monitoring well MW-7 was also abandoned on June 28, 2012. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services, Inc. (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1 summarizes the current and historical laboratory analytical results for microbiology and wet chemistry. All parameters analyzed, except fecal coliform and total organic carbon (TOC), were below the permit limits and North Carolina Administrative Code, Title 15A, Subchapter 2L.0202 Standards (21- Standards). Fecal coliform concentrations can be summarized as follows: •- The permit limit for fecal coliform is 1.0 colony forming units (CFU) per 100 milliliters (ml-) • The fecal coliform concentration in the MW-6 sample collected during this event (May 201-4) equals the permit limit and represents an increase from the previous sampling event (November 2013). • The fecal coliform concentration in MW-5 was below the laboratory reporting limit during this sampling event (May 2014) and represents a decrease from the previous sampling event (November 2013). • As in the November 2013 sampling event, the fecal coliform concentrations in MW-2, MW-4, MW-7R, and MW-8 remain below the laboratory reporting limit in the samples collected during this event (May 2014). • The laboratory reporting limit for sample MW-7R (10,000 CFU per 100 mL) was elevated and above the permit limit. • The laboratory reporting limit for samples MW-5 and MW-8 (10 CFU per 100 mL) was above the permit limit. P:\Clement Pappas\Reports\2014\May\Cover Letter—May.Doc Mr. Blake Kehoe June 23, 2014 Page 2 of 2 • The laboratory reporting limit for sample MW-4 (2 CFU per 100 mL) was above the permit limit. The TOC concentrations can be summarized as follows: • The permit limit for TOC is 10 milligrams per liter. • Similar to previous sampling events, the TOC concentration in the MW-8 sample collected during this event (May 2014) was in excess of the permit limit. • TOC concentrations in samples MW-2R, MW-4, MW-6, and MW-5 collected during this event (May 2014) have increased since the previous sampling event (November 2013). • TOC concentrations in samples MW-7R and MW-8 collected during this event (May 2014) decreased - since the previous sampling event (November 2013). Table 2 summarizes the current and historical groundwater elevations. All of the monitoring well concrete pads and top of the well casings elevations were resurveyed by Wes Cole Land Surveying, PA on November 21, 2012. The new elevations are reflected on Table 2 and on Figure 2. In accordance with the spray irrigation permit, Clement Pappas is required to submit by June 30, 2014 (on or before the last working day of the month following the sampling month) to the DENR the following: • Laboratory analytical report and chain -of -custody form (Attachment A); the well sampling logs and equipment documentation and instrument calibration data sheet is also included (Attachment B) • DENR GW-59A—Compliance Report Form (Attachment C) • DENR GW-59—Groundwater Quality Monitoring Compliance Report Form for each monitoring well (Attachment D) Altamont proposes to complete the next scheduled groundwater sampling event iri November 2014. The spray irrigation permit expires on November 30, 2015. If you have any questions or would like additional information, please do not hesitate to contact the undersigned at (828) 281-3350. If the answer to question 7 is "NO'; contact the Re_pional Office within 90 days; an evaluation may be required to determine the impact the waste disposal system is having at the review and compliance boundaries surrounding this facility. Failure to do so may subject the permittee to a Notice of Violation, fines, and/or penalties. Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCs and is possibly naturally occurring. Fecal coliform was detected in the May 2014 MW-6 sample at the standard but below historic detections. Fecal coliform was last detected in MW-5 during the November 2013 sampling event and in MW-4 and MW-8 during the May 2013 sampling event. g The person completing this portion (GW-59A) of the monitoring report should sign below and submit this form with GW-59 forms for required wells to the address provided at the top of the current GW-59 form. iiafereby ackno dge::tha e a eirifo"rin on as evaluae and the;mfolmafiori submitted m fhfs - repork (Coln fan, e p W 9 ;) istrue " nd om . lete to the best oflFn kriowled e'. s„,DIY ,_..P...........,...
