(Altamont) conducted a groundwater sampling event at your facility on -May 16, 2013. A site location map is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with the North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29,.2006 and reissued on October 12, 2012). Altamont collected groundwater samples from six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7R, and MW-8. Monitoring well MW-7R was installed on June 28, 2012 to replace MW-7, which was destroyed in June 2012 by field equipment. Monitoring well MW-7 was also abandoned on June28, 2012. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1 summarizes the current and historical laboratory analytical results for microbiology and wet chemistry. During this sampling event, the total organic carbon concentrations were in excess of the permit limit in MW-2R, MW-4, MW-7R and MW-8. The fecal coliform concentrations were in excess of the permit limit in MW-4, MW-5, MW-6, and MW-8. All other parameters analyzed were below permit limits and North Carolina Administrative Code, Title 15A, Subchapter 2L Standard (21- Standard), effective January 2010. Fecal coliform concentrations increased in MW-6 and MW-8 since the November 2012 sampling event. The fecal coliform concentration detected in MW-4 was the same as concentrations detected in the May and November 2012 sampling events. MW-5 contained a concentration at the detection limit and 2L Standard. The total organic carbon concentrations in MW-2R, MW-5, and MW-8 have decreased since the last sampling event. The concentration detected in MW-4, MW-6, and MW-7R increased since the November 2012 sampling event. The concentration detected in MW-4 and MW-7R remain above the associated permit limit. Table 2 summarizes the current and historical groundwater elevations. All of the monitoring well's concrete pads and top of the well casings elevations were resurveyed by Wes Cole Land Surveying, PA on November 21, 2012. The new elevations are reflected on Table 2 and on Figure 2 ---, RECEIVED JUL - c 12013 Asheville Regional Office "L:Iferrcie^cion P:\Clement Pappas\Reports\2013\May 2013\Cover Letter May.Doc ...__ _ _ e Mr. Blake Kehoe June 17, 2013 Page 2 of 2 In accordance with the spray irrigation permit, Clement Pappas is required to submit by June 30, 2013 (on or before the last working day of the month following the sampling month) to the DENR the following: o Laboratory analytical report and chain -of -custody form (Attachment A), the well sampling logs and equipment documentation and instrument calibration data sheet is also included (Attachment B) e DENR GW-59A—Compliance Report Form (Attachment C) o DENR GW-59—Groundwater Quality Monitoring Compliance Report Forms (Attachment D) Altamont proposes to complete the next scheduled groundwater sampling event in November 2013. (®; if the answer to question 7 is "YES", describe those actions in the space provided below. If the answer to question 7 is "NO" contact the Regional Office within 90 days; an evaluation may be required to determine the impact the waste disposal system is having at the review and compliance boundaries surroundinq this facility. Failure to do so may subject the permittee to a Notice of Violation, fines, and/or penalties. Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCs and is possibly naturally occurring. Fecal coliform was detected in the May 2013 samples MW-4, MW-5, MW-6 and MW-8 and contained concentrations above the November2012 results. Monitoring wells MW-5, MW-6, MW-7, and MW-8 were disinfected in February 2011. Fecal coliform was detected in MW-4 and MW-6 during the November 2012 sampling event. An investigation regarding the source and occurrence of the fecal coliforms exceedances was reported in October 2011 and is ongoing. 8 The person completing this portion (GW-59A) of the monitoring report should sign below and submit this form with GW-59 forms for required wells to the address provided at the top of the current GW-59 form.
