(Altamont) conducted a groundwater sampling event at your facility on May 17, 2012. A site location map is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with the North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006). Altamont collected groundwater samples from six monitoring wells: MW72R, MW-4, MW-5, MW-6, MW-7, and MW-8. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Table 1 summarizes the current and historical laboratory analytical results for microbiology and wet chemistry. Table 2 summarizes the current and historical groundwater elevations. During this sampling event, the total organic carbon concentrations were in excess of the permit limit in MW-6 and MW-8. The fecal coliform concentrations were in excess of the permit limit in MW-4, MW-6, and MW-8. All other parameters analyzed were below permit limits. In accordance with the spray irrigation permit, Clement Pappas is required to submit to the DENR the attached forms (DENR GW-59A—Compliance Report Form and DENR GW-59—Groundwater Quality Monitoring Compliance Report Forms) along with the laboratory analytical report and chain -of -custody form by June 30, 2012 (on or before the last working day of the month following the sampling month). Altamont proposes to complete the next scheduled groundwater sampling event in November 2012. The spray irrigation permit expires in November 30, 2015. YES NO • If the answer is "YES", a groundwater quality problem maybe occurring. CONTACT THE REGIONAL OFFICE IMMEDIATELY FOR GUIDANCE. If the answer is "NO", monitoring wells maybe improperly located; contact the Regional Office. 7 Is the permittee implementing previously approved actions required by the Division involving this YES NO groundwater quality problem? If the answer to question 7 is "YES", describe those actions in the space provided below. If the answer to question 7 is "NO" contact the Regional Office within 90 daVs; an evaluation may be required to determine the impact the waste disposal system is having at the review and compliance boundaries surrounding this facility. Failure to do so may subject the permittee to a Notice of Violation fines, and/or penalties. Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCs and is possibly naturally occurring. Fecal coliform was detected in the May 2012 samples MW-4, MW-6, and MW-8 and contained concentrations above the November 2011 results. Monitoring wells MW-5, MW-6, MW-7, and MW-8 were disinfected in February 2011. Fecal coliform was detected in MW-5, MW-6, and MW-8 during the November 2011 sampling event. An investigation regarding the source and occurrence of the fecal coliforms exceedances was reported in October 2011 and is ongoing. g The person completing this portion (G W-59A) of the monitoring report should sign below and submit this form with G W-59 forms for required wells to the address provided at the top of the current G W-59 form. 11 hereby acknowledge t f ab' e i atfon. as valuated and the information submitted m thisr report (Compha t e Retrie an o lete to the best of my -knowledge Signature 6f Permittee or utho d Agent) Date GW-59A 12/8/2003 4.,VX-j (/`7 mo,-,-- nl nnn CA nntl v u UNDWATER QUALITY MONITORING:, PLIANCE REPORT FORM or Type avmvu , Name: Clement Pappas Name (if different): Address: 125 Industrial Park :ndersonville ' (Street) NC 28792 County Henderson (City) (Stale) act Person: Mr. Blake Kehoe Telephone#: (828) 693-0711 Location/Site Name: Irrigation Area No. of wells to be sampled: 6 PERMIT Number: Expiration Date: 11/30/2015 Non -Discharge WQ0004797 UIC NPDES Other TYPE OF PERMITTED OPERATION BEING MONITORED ❑ Lagoon ❑ Remediation: Infiltration Gallery 0 Spray Field ❑ Remediation: ❑ Rotary Distributor ❑ Land Application of Sludge ❑ Water Source Heat Pump ❑ Other: If WELL WELL ID NUMBER (from Permit): MW-2R Date sample collected: 5/17/2012 Well Depth:' 14.90 ft. relative to top of casing Well Diameter: 2 in.
