(Altamont) conducted a groundwater sampling event at your facility on November 15, 2011. A site location map is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006). Altamont collected groundwater samples from -six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to Pace Analytical Services (Pace), a North Carolina certified laboratory, for analysis of the parameters specified in the permit. Representatives from DENR, Mr. Jonathan Stepp and Brett Laverty, were on -site and collected split samples from the monitoring wells. Table 1 summarizes the current and historic laboratory analytical results for microbiology and wet chemistry. The concentration of total organic carbon was in excess of the permit limit in the sample collected from well MW-2R. The concentrations of fecal coliform were in excess of the permit limits in the samples collected from wells MW-5 and MW-6. All other parameters analyzed were below permit limits. Fecal coliform was elevated in some samples in November 2010. Altamont performed maintenance on the monitoring wells and the wells were disinfected with sodium hypochlorite in February 2011. Additional assessments were conducted to determine the source of the fecal coliform. Details concerning the assessments conducted at the plant, lagoon, spray field, and monitoring wells to identify the source and occurrence of the fecal coliform is addressed in the Coliform Contamination Investigation report submitted to the DENR Aquifer Protection Section on October 4, 2011. Fecal coliform concentrations increased in MW-5 and MW-6 and decreased in MW-8 since the May 2011 sampling event (MW-8 was below laboratory reporting limits in November 2011). The concentrations detected in MW-5 and MW-6 are similar to concentrations detected in the November 2010 sampling event. The total organic carbon concentrations in MW-4, MW-5, MW-6, MW-7, and MW-8 have decreased since the last sampling event. The concentration detected in MW-2R increased since the May 2011 sampling event and remains above the associated permit limit but the concentration detected is similar to the November 2010 sampling event concentration. Table 2 summarizes the current and historic laboratory analytical results for volatile organic compounds. Concentrations of tetrachloroethene, trichloroethene, and cis-1,2-dichloroethene were detected in excess of the North Carolina Administrative Code, Title 15A, Subchapter 2L Standard (2L Standard) in well MW-5. The detection does not exceed the 2L Standard of 70 pg/L. All other parameters analyzed were below 2L Standards. Table 3 summarizes the current and historical groundwater elevations. In accordance with the spray irrigation permit, Clement Pappas is required to submit to the DENR the attached forms (DENR GW-59A—Compliance Report Form and DENR GW-59—Groundwater Quality Monitoring Compliance Report Forms) along with the laboratory analytical report and chain -of -custody form by December 31, 2011(on or before the last working day of the month following the sampling month). Altamont proposes to complete the next scheduled groundwater sampling event in May 2012. The spray irrigation permit expires in November 30, 2014. In accordance with the Compliance Evaluation Inspection letter issued by Ms. Beverly Price of the DENR, Division of Water Quality (DWQ) on April 27, 2007, volatile organic compounds and semivolatile organic compounds were sampled during the May 2007 sampling event. 8. Napthalene was detected in MW-6 for the first time at a concentration of 2.2 pg/L during the November 2002 sampling event, which is below the.2L Standard of 6 pg/L. 9. 1,1,2-trichloroethane was detected in MW-5 for the first time on November 20, 2010 at a concentration of 8.0 pg/L, which exceeds the North Carolina Interim Maximum Allowable Concentration of 0.6 pg/L. 10. Chloroform was detected for the first time in MW.5 at a concentration of 31.3 pg/L during the November 2011 sampling event, which is below the 2L Standard of 70 pg/L.
