ALTA M O NTENVI RON M E NTA L.COM December 17, 2010 Mr. Blake Kehoe Clement Pappas Post Office Box 1009 Mountain Home, North Carolina 28758 Subject: November 2010 Groundwater Sampling Event Clement Pappas 125 Industrial Park Road Mountain Home, North Carolina Dear Mr. Kehoe, JAN 19 2G 11 Asheville Regional Office Altamont Environmental, Inc. (Altamont) conducted a groundwater sampling event at your facility on November 81, 2010. A site location map is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006). Altamont collected groundwater samples from six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to a certified laboratory for analysis of the parameters specified in the permit. Representatives from the DENR, Mr. Jonathan Stepp and Brett Laverty, were onsite and collected split samples from the monitoring wells. Table 1 summarizes the current and historic laboratory analytical results for microbiology and wet chemistry. The concentration of total organic carbon was in excess of the permit limit in the sample collected from wells MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8. The concentration of fecal coliform was in excess of the permit limits in the sample collected from wells MW-5, MW-6, and MW-8. Table 2 summarizes the current and historic laboratory analytical results for volatile organic compounds. Concentrations of tetrachloroethene, trichloroethene, and cis-1,2- dichloroethene were in excess of the North Carolina Administrative Code, Title 15A, Subchapter 2L Standard (2L Standard) in well MW-5. MW-5 contained a detection of 1,1,2-trichloroethene for the first time at a concentration of 8.0 micrograms per liter (µg/L). The detection exceeds the North Carolina Interim Maximum Allowable Concentration of 0.6 µg/L. A 2L Standard does not exist for this compound. All other parameters analyzed were below permit limits. In addition to the Figures, Table 1, and Table 2, please find attached the following: • Table 3—Current and Historical Groundwater Elevation Measurements m The laboratory and analytical report and chain -of -custody form AltamonVs Well Sampling Logs and Equipment Documentation and Instrument Calibration Data Sheet • One DENR GW-59A—Compliance Report Form ® Six DENR GW-59—Groundwater Quality Monitoring: Compliance Report Forms P:\Clement Pappas\Reports\2010\November 2010\Cover Letter November.Doc Mr. Blake Kehoe December 17, 2010 Page 2 of 2 In accordance with the permit, Clement Pappas is required to submit to the DENR the attached forms along with the laboratory analytical report and chain -of -custody form by December 31, 2010 (on or before the last working day of the month following the sampling month). Fecal coliform exceedences remain in MW-5, MW-6, and MW-8 and have increased since the May 2010 sampling event. The laboratory used the filter count analyzing, method during this event. The tube analyzing method was used during the May 2010 and November 2009 sampling events. The laboratory performs the filter count method now because the method bases the results on actual colony counts instead of an estimation of how many tubes go positive. The total organic carbon and fecal coliform concentrations in MW-5, MW-6, and MW-8 have increased since the last sampling event. MW-5 is located upgradient of field #2, near the weigh station along Interstate 26. Altamont proposes to complete the next scheduled groundwater sampling event in May 2011. If you have any questions or would like additional information, please do not hesitate to contact the undersigned at (828) 281-3350. ' Respectfully yours, ALTAMONT ENVIRONMENTAL, INC. Total Organic Carbon, VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCs. Possibly naturally occurring. The November 2010 groundwater samples were analyzed according to a fecal coliform filter count method, the same as the May 2009 sampling event. The November 2009 and May 2010 groundwater samples were analyzed according to the fecal coliform tube method. Fecal coliform was detected in samples MW-5, MW-6, MW-6 and contained concentrations above the May 2010 results. Representatives of the DENR Aquifer Protection Section collected split samples from the monitoring wells. g The person completing this portion (G W-59A) of the monitoring report should sign below and submit this form with G W-59 forms for required wells to the address provided at the top of the current G W-59 form. l hereby acknowledge that he above information was evaluated and the infortnatlon submitted in this report (Compliance: Report QWv9A) is true and complete, o theybest of my knowledge MX1 ' Signature of Permittee (or Authorized Agent) Date GW-59A 12/3/2003 SUBMIT FORM ON YELLOW PAPER ONLY GROUNDWATER QUALITY MONITORING: DEPARTMENT OF ENVIRONMENT 8 NATURAL RESOURCES,--" DIVISION OF WATER QUALITY INFORMATION PROCESSING UNIT COMPLIANCE REPORT FORM • • • 1617 MAIL'SERVICE:CENTER, RALEIGH NC 27fi99 1617 Phone (919) 733 3221 ;" FACILITY INFORMATION Please Print Clearly or Type PERMIT Number: Expiration Date: 11/30/2011 Facility Name: Clement Pappas Non -Discharge WO0004797 UIC Permit Name (if different): NPDES Other Facility Address: 125 Industrial Park TYPE OF PERMITTED OPERATION BEING MONITORED Mountain Home (Street) NC 28758 County Henderson ❑ Lagoon . ❑ Remediation: Infiltration Gallery (City) (State) (zip) M Spray Field ❑ Remediation: Contact Person: Mr. Blake Kehoe Telephone#: (828) 693-0711 ❑ Rotary Distributor ❑ Land Application of Sludge Well Location/Site Name: Irrigation Area No. of wells to be sampled: 6 ❑ Water Source Heat Pump ❑ Other: (from Permit SAMPLING INFORMATION If WELL WELL ID NUMBER (from Permit): MW-2R Date sample collected: 11/8/2010 FIELD ANALYSES: WAS Well Depth: 1 14.90 ft.
