RECF Li JUN 2 5 2010 Information i-�,ro(;essing Unit DVVQ/,?OG Altamont Environmental, Inc. (Altamont) conducted a groundwater sampling event at your facility on May 13, 2010. A, site location map is included as Figure 1. Altamont conducted the groundwater sampling event in accordance with North Carolina Department of Environment and Natural Resources (DENR) spray irrigation system permit number WQ0004797 (permit issued December 29, 2006). Altamont collected groundwater samples from six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8. The locations of the monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to a certified laboratory for analysis of the parameters specified in the permit. The fecal coliform groundwater samples collected from MW-7 and MW-8 on May 13, 2010 were not analyzed by t-he laboratory within the holding time, so new samples were collected from the two wells on May 21, 2010 for fecal coliform analyses. Table 1 summarizes the current, and historic laboratoryanalytical results for microbiology and wet chemistry. The concentration of total organic carbon was in excess of the permit limit in the sample collected from well MW-8. The concentration of fecal coliform was in excess of the permit limits in the sample collected from well MW-5. All other parameters analyzed were below permit limits. In addition to the Figures and Table 1, please find attached the following: • Table 2 - Current and Historical Groundwater Elevation Measurements, • The laboratory and analytical report and chain of custody form, _ • Altamont's well sampling logs and equipment documentation and instrument calibration data sheet, • One DENR GW-59A - Compliance Report Form, and • Six DENR GW-59 - Groundwater Quality Monitoring: Compliance Report Forms. PAClement Pappas\Reports\2010\May 2010\Cover Letter May.Doc Mr. Blake Kehoe June 10, 2010 Page 2 of 2 In accordance with the permit, Clement Pappas is required to submit to the DENR the attached forms along with the laboratory analytical report and chain of custody form by June 30, 2010 (on or before the last working day of the month following the sampling month). Due to the previous.exceedences of fecal coliform in MW-5, MW-6, MW-7, and MW-8, Altamont instructed the laboratory to utilize the tube analyzing method during this sampling event and the previous sampling event in November 2009. Concentrations were much higher during the May 2009 sampling event where the filter count method was used. Fecal coliform concentrations in MW-6, MW-7, and MW-8 during this sampling event were below laboratory detection limits and the respective standard. The concentration in MW-5 had increased since the last sampling event. The well is located upgradient of field #2, near the weigh station along Interstate 26. Altamont proposes to complete the next scheduled groundwater sampling event in November 2010. Altamont will continue to monitor fecal coliform using the tube method. Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The TOC is not attributable to VOCs or SVOCs and is possibly naturally occurring. Fecal coliform was detected in the November 2011 samples MW-5 and MW-6 and contained concentrations above the May 2011 results. Monitoring wells' MW-5, MW-6, MW-7, and MW-8 were disinfected in February 2011. Fecal coliform was only detected in MW-8 during the May 2011 sampling event. An investigation regarding the source and occurrence of the fecal coliforms exceedances was reported in October 2011 and is ongoing.
