The November 2009 groundwater samples were analyzed according to an attemative fecal coliforrn method. Fecal colifonn was still detected in the above monitoring wells but contained concentrations below the May 2009 results. Fecal colifonn results will be reviewed following the May 2010 monitoring event to verify that they continue to decrease. g The person completing this portion (GW--59A) of the monitoring report should sign below and submit this form with G W-59 forms for required wells to the address provided at the top of the current G W-59 form. 1 hereby acknowledge thatythe above mformatton was evaluated and the rnformaton submlttec]in thts Y: report (Colrnpir ce Report.GW-59A) is true and completeao the best of (my knowledge' , _ tuna We f Permittee (or Authorized Agent) Date GW-59A 12/8/2003 SUBMIT FORM ON YELLOW PAPER ONLY QUALITY MONITORING: b iYA§ffl,, N ,gg,GROUNDWATER ""vi 0 COMPLIANCE REPORT FORM sc e, z;' FACILITY INFORMATION Please Pnnt Clearly or Type PERMIT Number: Expiration Date: 11/30/2011 Facility Name: Clement Pappas Non -Discharge WQ0004797 UIC Permit Name (if different): NPDES Other Facility Address: 125 Industrial Park TYPE OF PERMITTED OPERATION BEING MONITORED Mountain Home, (street) NC 28758 County Henderson ❑ Lagoon ❑ Remediation: Infiltration Gallery (city) (state) (zip) x❑ Spray Field ❑ Remediation: Contact Person: Mr. Blake Kehoe Telephonet (828) 693-0711 ❑ Rotary Distributor ❑ Land Application of Sludge Well Location/Site Name: Irrigation Area No. -of wells to be sampled: 6 ❑ Water Source Heat Pump ❑ Other: from Permit SAMPLING INFORMATION If WELL WELL ID NUMBER (from Permit): MW 2R Date sample collected: 11/5/2009 FIELD ANALYSES: WAS Well Depth: 14.90 ft. relative to top of casing Well Diameter: 2 in. pH 5.62 uni Temp. 16.66 eC DRY at Depth to Water Level: 6.72 ft. below measuring point Screened Interval: 6 ft. to 11 ft."
