Enter date monitoring results were due. (12/31/20081 Will this monitoring report (GW-59 and GW-59A) be submitted after the established due date? Was any required information missing on the GW-59 report forms? IF the answer to question 1 or 2 is "YES", list in the space provided below the well identification number(s) and explain the problems encountered in obtaining the required information. Are any of the monitor wells in need of repair or maintenance (damaged casing, unlocked or missing cap, missing YES identification plate, area overgrown, etc.)? Failure to do so may subject the permittee to a Notice bf V1616fioi► fines, and/orpenaldes. Total Organic Carbon (TOC) was present in samples collected from MW-4, MW-5, and MW-8. The TOC concentration in MW-7 a ri kta le to volatile organic compounds (VOCs). Based upon historic VOC and semi -volatile organic compound (SVOC) analyses, the TOL bn nt ti( ir%O f R MW-4 and MW-8 is likely naturally occurring. No action is currently being taken with respect to fecal coliform. If subsequent sampling events indicate the continued presence_of this.- constituent in groundwater, further action may be taken. ` t "-% '- "Sai•.'.;r According to a meeting with Mr. Qu Qi of the Aquifer Protection Section on March 8, 2002, the VOCs present in MW-5, MW-6, and M' W-7 are from an offsite source uphill from MW-5, possibly the North Carolina Department of Transportation. g The -person' completing this portion (GW,--59A) of the monitoring report should sign below and submit this fonn with GW-59 forms for required wells to the address provided at the top of the current GW-59 form. acGiovyie`ge"$iat tie abpye Inforlrtattpnwa` evaua a apc tFte tnformabor submiltted m this grepg%pahCe.RepoGW-59)is true anccomplete to the best of mGimow_led9q,' 4 i tir Fx µ kr .
