(12/31/20081 Will this monitoring report (GW-59 and GW-59A) be submitted after the established due date? Was any required information missing on the GW-59 report forms? IF the answer to question 1 or 2 is "YES", list in the space provided below the well identification number(s) and explain the problems encountered in obtaining the required information. Are any of the monitor wells in need of repair or maintenance (damaged casing, unlocked or missing cap, missing YES identification plate, area overgrown, etc.)? Ifthe answer is "Yes", contact the Regional Ofce for guidance. 4 Are any monitored constituents equal to or above the established standards? NO If the answer to question 4 is "NO'; skip to section 8. If the answer to question 4 is "YES" list the affected wells individually with constituent(s) and concentrafion(s) exceeding standards in the space provided below. Total Organic Carbon (limit =10 mg/L) - . MW-4 = 20.5 mg/L, MW-5 = 25.9 mg/L, and MW-8 =14.4 mg/L Fecal Coliform (limit =1 /100 mL) - MW-8 = 2.2/100 mL Tetrachloroethene (limit = 0.7 pg/L) - MW-5 = 2680 pg/L, MW-6 = 3 pg/L, and MW-7 = 0.79 pg/L Trichloroethene (limit = 2.8 pg/L) - MW-5 = 534 µg/L cis-1,2-dichloroethene (limit = 70 pg/L) - MW-5 = 293 µg/L 5 For the constituents identified in question 4 above, have standards been exceeded previously for the ES NO same constituent(s) in the same well(s) in the last two years? If the answer to question 5 is "NO", skip to section S. CONTACT THE REGIONAL OFFICE IMMEDIATELY FOR GUIDANCE. if the answer is "NO", monitoring wells maybe improperly located; contact the Regional Office. 7 Is the permittee implementing previously approved actions required by the Division involving this S NO groundwater quality problem? If the answer to question 7 is "YES, describe those actions in the space provided below. If the answer to question 7 is "NO" contact the Repiona/Office within 90 days an evaluation may required to determine the Impact the waste disposal system Is havino at the review and compliance boundaries surroundm this facility. Failure to do so may subject the permittee to a Notice bf V1616fioi► fines, and/orpenaldes. Total Organic Carbon (TOC) was present in samples collected from MW-4, MW-5, and MW-8. The TOC concentration in MW-7 a ri kta le to volatile organic compounds (VOCs). Based upon historic VOC and semi -volatile organic compound (SVOC) analyses, the TOL bn nt ti( ir%O f R MW-4 and MW-8 is likely naturally occurring. No action is currently being taken with respect to fecal coliform. If subsequent sampling events indicate the continued presence_of this.- constituent in groundwater, further action may be taken. ` t "-% '- "Sai•.'.;r According to a meeting with Mr. Qu Qi of the Aquifer Protection Section on March 8, 2002, the VOCs present in MW-5, MW-6, and M' W-7 are from an offsite source uphill from MW-5, possibly the North Carolina Department of Transportation. g The -person' completing this portion (GW,--59A) of the monitoring report should sign below and submit this fonn with GW-59 forms for required wells to the address provided at the top of the current GW-59 form. acGiovyie`ge"$iat tie abpye Inforlrtattpnwa` evaua a apc tFte tnformabor submiltted m this grepg%pahCe.RepoGW-59)is true anccomplete to the best of mGimow_led9q,' 4 i tir Fx µ kr .
