Altamont Environmental, Inc. (Altamont) conducted a groundwater sampling event at your facility on May 17,
2011. A: site location map is included as Figure 1. Altamont conducted the groundwater sampling event in
accordance with North Carolina Department of Environment and Natural Resources (DENR) spray irrigation
system permit number WQ0004797 (permit issued December 29, 2006). Altamont collected groundwater
samples from six monitoring wells: MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8. The locations of the
monitoring wells are shown on Figure 2 (attached). Altamont submitted the groundwater samples to a North
Carolina certified laboratory for analysis of the parameters specified in the permit.
Table.1 summarizes the current and historic laboratory analytical results for microbiology and wet chemistry.
The concentration of total organic carbon was in excess of the permit limit in the sample collected from wells
MW-2R and equaled the permit limit in MW-8. The concentrations of fecal coliform and nitrate were in
excess of the permit limits in the sample collected from well MW-8. All other parameters analyzed were
below permit limits.
Fecal coliform was also elevated in some wells in November 2010. In order to investigate the source of the
fecal coliform in monitoring wells MW-5, MW-6, MW-7, and MW-8 during the November 2010 sampling
event, Altamont performed maintenance on the monitoring wells. The wells were disinfected with sodium
hypochlorite in February 2011. Further details will be presented in a forthcoming report concerning the
assessments conducted at the plant, lagoon, spray field, and monitoring wells to identify the source and
occurrence of the fecal coliform.
Fecal coliform exceedances decreased in MW-5, MW-6, and MW-8 since the November 2010 sampling
event (MW-5 and MW-6 were below laboratory reporting limits). The laboratory used the same filter count
analyzing method during the May 2011 and November 2010 sampling events. T he tube analyzing method
was used during the May 2010 and November 2009 sampling events. The laboratory performs the filter
count method now because the method bases the results on actual colony counts instead of an estimation
of how many tubes become positive.
The total organic carbon concentrations in MW-2R, MW-4, MW-5, MW-6, MW-7, and MW-8 have also
decreased since the last sampling event. Total dissolved solids analytical results for MW-2R and MW-5
contained..elevated reporting limits during this event. These elevated levels are likely due to the laboratory
using -less -of the sample during testing because of difficulty filtering the correct amount. Historically, total
dissolved) solids results for these monitoring wells have been below the North Carolina Administrative Code
(NCAC), Title-15A, Subchapter 2L (21- Standard) and have shown a decrease in Concentrations overtime.

In accordance with the spray irrigation permit, Clement Pappas is required to submit to the DENR the
attached forms along with the laboratory analytical report and chain -of -custody form by June 30, 2011 (on or
before the last working day of the month following the sampling month).
Altamont proposes to complete the next scheduled groundwater sampling event in November 2011. The
spray irrigation permit expires in November 30, 2015.

Total Organic Carbon (TOC), VOC, and SVOC analyses were completed for all samples during the May 2007 groundwater sampling event. The
TOC is not attributable to VOCs or SVOCs and is possibly naturally occurring. Fecal coliform was detected in the November 2010 samples MW-5,
MW-6, MW-8 and contained concentrations above the May 2010 results. Monitoring wells MW-5, MW-6, MW-7, and MW-8 were disinfected with
chlorine in February 2011. Fecal coliform was only detected in MW-8 during the May 2011 sampling event. An investigation regarding the source
and occurrence of the fecal coliforms exceedances is ongoing.
