in accordance with 15A NCAC 02T .0604(h). The Operation and Maintenance Agreement
submitted with this application was only signed by one of the two deeded property owners.
Operation and Maintenance Agreements must be signed by all deeded property owners. Resubmit
this form with all required signatures.
