Based upon our previous discussions, the design flow of 4,131 GPD for this proposed facility is based upon a 100 GPD/1000 sf design flow rate. The design sewer flow for this project (as shown in Section B.9 of the FTA 08-13 Application form) was calculated as follows: Total Building Area = 41,117 sf+ 192 sf= 41,309 sf Design Flow = (41,309/1000) x 100 = 4,130.9 GPD use 4,131 GPD This project has been reviewed and approved with the local sewer authority (City of Goldsboro) and has been designed in accordance with their standards. Downstream Sewer, WWTF Capacity and Flow Tracking/Acceptance Form (FTSE 08-13) • Submit the completed and appropriately executed Flow Tracking/Acceptance for Sewer Extension Permit (FTSE 08-13) Form for all applications. ➢ The applicant (and owners of downstream sewers, pump stations and/or treatment facilities submitting form FTSE 08-13 certifies that the addition of the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving treatment plant, and that the flow from this project will not cause capacity related sanitary sewer overflows or overburden any downstream pump station en route to the receiving wastewater treatment plant. ➢ Where the applicant is not the owner of the downstream sewer, submit two copies of form FTSE 08-13 from the owner of the downstream sewer and owner of the WWTF, if different. ➢ The flow acceptance indicated in form FTSE 08-13 must not expire prior to permit issuance and must be dated less than one year prior to the application date. ➢ Submittal of this application and form FTSE 08-13 indicates that owner has adequate capacity and will not violate G.S. 143-215.67(a). • Intergovernmental agreements or other contracts will not be accepted in lieu of a project - specific FTSE 08-13. G. Site Map (All Application Packages): ➢ Submit an 8.5-inch x 11-inch color copy of a USGS Topographic Map of sufficient scale to identify the entire project area and closest surface waters. Each map must include at a minimum: ➢ The location of the sewer line and pump stations and be of reproducible quality. ➢ Downstream connection points and the permit number for the receiving sewer (if known) ➢ Pump Station Locations and the longitude and latitude for each pump station (if applicable) p Include a street level map showing all relevant project areas. H. Stream Classification (WSCAS 08-13) p Submit the completed and appropriately executed Watershed Classification Attachment form (WSCAS 08-13) if any portion of the sewer system project is within 100 feet of any surface water or wetlands. ➢ A variance must be requested for encroachment within required setbacks or buffers pursuant to 15A NCAC 02T .0305(f) I. Environmental Assessments (Projects subject to an Environmental Assessment (EA)): ➢ Projects involving an Environmental Assessment per 15A NCAC 01C .0408, must be submitted for a full technical review and must be submitted to the PERCS Unit on application forms provided by the Division. J. Alternative Sewer Systems • Projects involving low pressure sewer systems, vacuum sewer systems and other alternative sewer systems must be submitted for a full technical review and must be submitted to the PERCS Unit on application forms provided by the Division. K. Flow Direction ➢ Many wastewater treatment systems are entering into agreements for regionalization efforts and emergency treatment capacity. Parts of the system are installed so that the wastewater flow can be directed to more than one treatment facility. If this is the case with the project, please indicate in B(12) and give the permit number of the second facility. L. Certifications — Section C The application must be certified by both the applicant and the design engineer who is a North Carolina Registered Professional Engineer (PE). The applicant signature must match the signing official listed in Item A(1b). The PE should NOT certify the application if he/she is unfamiliar with 15A NCAC Chapter 2T, The Gravity Sewer Minimum Design Criteria (most recent version) and the Minimum Design Criteria for the Permitting of Pump Stations and Force Mains (most recent version), as applicable to the project. Based on discussions with DENR, it was determined that the 130 GPD/1000 sd (Shopping centers & Malls with food service) daily design flow would be excessive for this application 10. WWTP Facility Name: Enter the name of the WWTP that will receive the wastewater flow. b. WWTP Facility Permit #: Enter the NPDES or Non -Discharge number for the WWTP receiving the wastewater flow. c. WWTP facility's permitted flow, MGD: From WWTP owner's NPDES or Non -Discharge permit. d. For POTWs that implement a pretreatment program, include flows allocated to industrial users who may not be using all of their flow allocation. Please contact your Pretreatment Coordinator for information on industrial flow tributary to your WWTP. As of January 15, 2008 the POTW should have reviewed flow allocations made over the last two years and reconciled their flow records, to the best of their ability, so it is known how much flow has been obligated and is not yet been made tributary to the WWTP, in accordance with local policies and procedures employed by the reporting entity. The obligated flow not yet tributary plus actual flow will be reconciled annually for systems at less than 60% of permitted flow used. Annual updates shall be submitted to the appropriate Regional Office by January 15 of each year and cover the previous calendar year. Semi -Annual updates shall be required when the percent of permitted flow used reaches 60%. Semi- annual time period are defined as January 1 through June 30 and July 1 through December 31. Semi- annual updates shall be submitted to the appropriate Regional Office by July 15 and January 15 of each year. Quarterly updates shall be required when the percent of permitted flow used reaches 80%. Quarters are defined as Qtrl (Jan -Feb -Mar); Qtr2 (Apr -May -Jun); Qtr3 (Jul -Aug -Sep); Qtr4 (Oct -Nov -Dec). Quarterly updates shall be submitted to the appropriate Regional Office by April 15, July 15, October 15 and January 15 of each year. e. Verify that trench cut and excavation is ready to receive work and excavations, dimensions, and elevations are as indicated on Construction Drawings. 3.2 PREPARATION A. Hand trim excavations to required elevations. Correct over excavation with bedding material. B. Remove large stones or other hard matter that could damage pipe or impede consistent backfilling or compaction. 3.3 BEDDING A. Excavate trench and place bedding material in accordance with Section 02300. 3.4 INSTALLATION - PIPE A. Install type and class of pipe as shown on the drawings. Pipes shall be laid and maintained to the required line and grade with necessary fittings, bends, manhole risers, cleanouts and other appurtenances placed at the required loca- tions. The pipe shall be installed with uniform bearing under the full length of the barrel of the pipe. The pipe shall be inspected for defects and cracks before being lowered into the trench. Defective, damaged or unsound pipe, or pipe that has had its grade disturbed after laying shall be taken up and replaced. Commence installation at lowest point with the bell end upgrade. B. No pipe shall be laid in water or when trench conditions are unsuitable for work. C. Pipe connecting to manholes or other structures shall terminate flush inside of the structure wall. D. Joints for PVC and LISP shall be thoroughly lubricated with an approved lubricant before pipe sections are slipped together. Open ends shall be fully protected with a stopper to prevent earth or other material from entering the pipe during construction. Carefully free interior of the pipe from dirt, cement and other deleterious material as the work progresses. E. Maintain separation of potable water main from sewer piping at crossings a minimum of 10 feet horizontal and 18 inches vertical. F. Install HDPE piping and fittings to AWWA C901 and C906. Butt fusion welded per ASTM D3261. G. Route pipe in straight line parallel to roads, buildings and adjacent utilities and as shown on the drawings. H. Establish elevations of buried piping with sufficient cover as recommended by pipe manufacturer to ensure not less than 3 feet of cover, except as noted on drawings. I. Form and place concrete for thrust blocks at each elbow of pipe force main. See construction drawing for details of construction. J. Backfill trench in accordance with Section 02300. K. Install trace wire continuous over top of non-metal pipe. Bury 6 inches minimum below finish grade, above pipe- line. 3.5 INSTALLATION — CLEANOUTS AND MANHOLES 02535-3 Goldsboro (N), NC — Store No. 7340-00 04/14/14 A. Form bottom of excavation clean and smooth to correct elevation. B. For cleanouts, form and place cast -in -place concrete base pad with provision for sanitary sewer pipe to be installed to proper elevations. C. For manholes, construct inverts according to the following guidelines: 1. Invert channel shall be smooth and accurately shaped to a semicircular bottom to match with the inside of the adjacent sewer section. 2. Invert channels and structure bottoms shall be shaped with mortar and lean concrete. 3. Changes in size and grade of invert shall be made gradually and evenly. 4. Changes in the direction of the sewer entering branch or branches shall have a true curve of as large a radi- us as the manhole will permit. D. For manholes, provide manhole rings, frame, and cover as shown on the construction drawings. 3.6 FIELD QUALITY CONTROL A. Field quality control shall be conducted by the Contractor in accordance with Section 01452. B. Pipes and joints shall not be completely backfilled until after inspection, testing, and approval by the Owner and Local jurisdiction. C. Prior to testing for leakage, the pipe trench shall be backfilled to at least the spring line of the pipe. If required to prevent pipe movement during testing, additional backfill shall be added leaving the pipe joints uncovered to per- mit inspection. D. Exfiltration Test 1. Each section of sewer line between successive manholes shall be tested by closing the lower end of the sewer to be tested and the inlet sewer of the upper manhole, using stoppers. 2. Fill the manhole and pipe with water to a point which produces a maximum of 3 feet of head above the in- vert of the sewer at the center of the upper manhole; or if groundwater is present, 3 feet of head above the average adjacent groundwater level. 3. The allowable leakage shall be 200 gal/inch of pipe diameter/mile/day E. Infiltration Test 1. If excessive ground water is encountered in the construction of a section of the sewer, the exfiltration test shall not be used. 2. The upper and lower ends of the sewer to be tested shall be closed sufficiently to prevent the entrance of water. 3. Pumping of ground water shall be discontinued for at least 3 days; then infiltration shall be tested. 4. Infiltration into each section of sewer between adjoining manholes shall not exceed that allowed for the ex - filtration test, except that head conditions shall be a maximum of 6 feet. F. The Exfiltration Test may be limited to the manholes only when the authority having jurisdiction does not require the test and the construction manager waives the test. The Infiltration Test will always be required when excessive ground water is encountered in addition to the air test. G. Air Test: Gravity systems shall be air tested between manholes at 3.5 psi for 5 minutes per ASTM F1417 for plas- tic pipes. H. Deflection Test: 1. Deflection tests shall be conducted on all plastic pipe using a mandrel with a diameter equal to 95 percent of the inside diameter of the pipe. The test shall be performed without mechanical pulling devices. 2. Allowable Deflection: Maximum allowable pipe deflection shall not exceed 5 percent of nominal inside diameter. 3. Mandrel: Mandrel, go/no-go, device shall be cylindrical in shape and constructed with either 9 or 16 even- ly spaced arms or prongs. Mandrels with fewer arms will be rejected as not sufficiently accurate. Procedure: Mandrel shall be hand -pulled through flexible pipe sewer lines no earlier than 30 days after trench has been completely backfilled. Sections of sewer not passing mandrel shall be uncovered and rebedded, rerounded, or replaced to satisfaction of Owner or governing agency. Repaired section shall be retested.
