Contractor is fully responsible for contacting appropriate parties and assuring that existing utilities are located prior to beginning construction. 2. Contractor is responsible for placing barricades, using flag men, etc. as necessary to ensure the safety of the public. 3. All pavement cuts, asphalt or concrete, are to be placed according to standards of the North Carolina Department of Transportation and City of Monroe specifications. 4. Shoring will be according to OSHA Trenching Standards Part 1926, Subpart P, or as amended. 5. All deeds, easements and encroachment agreements necessary for installation and operation of the system shall be obtained prior to operation of the system. 1. All roadway construction methods shall be in accordance with the City of Monroe Standard Specifications and Detail Manual and the NCDOT Standard Specifications for Roads and Structures, latest edition except when otherwise noted. 2. All storm drain pipes shall be placed to proper grade and alignment per approved plans. 3. No cracked or broken pipes shall be placed or accepted in construction of the project. 4. All catch basins 3'-6" deep or more shall have steps placed at 1'-2" on center. 5. Erosion control measures shall be placed around drainage inlets and catch basins. 6. All pipes in storm drainage structures shall be cut off flush with the inside wall. 7. All structures shall be smooth inside and outside of the structure using mortar mixed to manufacturer's specifications to avoid debris build-up obstructing flow. 8. All tail ditches shall have positive drainage with a minimum 1.0% slope. 9. The developer shall obtain any off -site drainage easement from adjacent property owners required to complete the improvements or provide positive drainage flow prior to construction plan approval. 10. All concrete for drainage structures shall be a minimum of 3600 psi except where otherwise noted or required. 11. If the developer wished to use an re -cast storm drainage structures within a proposed street right-of-way,he must submit a Professional Engineer P Y P g P P 9 certification for the structure. All structures must be certified to adequately carry H2O loading. Prior approval from the Engineering Director shall be obtained in order to incorporate ANY pre -cast storm drainage structures in a proposed street right-of-way. 12. Under no circumstances shall a pre -cast storm structure be cut or altered without a Professional Engineer's Certification of adequate load bearing strength following the alteration of the structure, except where designated "knock -out" panels are provided. 13. Under no circumstances shall water be permitted to rise in un-backfilled trenches after the pipe has been placed. 14. The laying of pipe in the finished trench shall be started at the lowest point and laid upgrade. The bell shall be laid upgrade. 15. Survey staking and proposed grades shall be consistent with plans and field conditions. 16. No construction activities are allowed to take place beyond the floodplain line or the stream buffer line, without a permit issued by the U.S. Army Corps of Engineers, the North Carolina Department of Environment and Natural Resources, and the City of Monroe. The cost to rectify any adverse situation to meet the City of Monroe Standard Specifications and Detail Manual and the City of Monroe ordinances shall be borne solely by the developer, the engineer, or contractor as legally required. All dimensions are at 90 degrees unless otherwise noted. 2. Contractor shall establish and verify point of beginning (P.O.B) and stake sire as indicated on construction documents prior to commencement of construction. Notify Merrick & Company immediately of any discrepancies. 3. All dimensions are to edge of pavement or centerline unless otherwise noted. 4. All construction shall be in accordance with City of Monroe Standard Specifications and Detail Manual. 5. All radii shall be 30' back of curb (32' edge of pavement) unless otherwise noted. 6. All sidewalks shall be 5'-0" unless otherwise noted. 7. Parking space dimensions are 18.0' x 9.0' wide unless otherwise noted. 8. No obstruction of public R/W or sidewalk will be allowed. No parking is allowed over public sidewalks. 9. No demolition landfills allowed on site. 10. The purpose of the storm drainage easement (SDE) is to provide storm water conveyance. Structures or any other object which may impede storm water flow or system maintenance are prohibited. Maintenance of the site shall be the responsibility of the home owner. 11. Approval of this plan is not an authorization to grade adjacent properties. When conditions warrant off -site grading, permission must be obtained from the affected property owners. 12. In order to assure proper drainage, keep a minimum of 0.5% slope on the curb. 13. Subsurface drainage facilities may be required in the street right-of-way if deemed necessary by the inspector. 14. The developer shall maintain each stream, creek, or backwash channel in an unobstructed state and shall remove from the channel and banks of the stream all debris, logs, timber, junk and other accumulations. 15. Handrails may be required by a representative of the city and/or NCDOT, if warranted by field conditions. 16. High -Density polyethylene (HDPE) storm drainage pipe not allowed within the road right of way per section 04.02.01 of the City of Monroe Standard Specifications and Detail Manual. 17. Per bullet 1 under 04.02.01 (Design Criteria) in the City of Monroe's Standard Specifications and Design Manual, all pipes within the Rlght-of-Way shall be RCP. Pipe outside of the street Right -of -Way may be HDPE. 18. All openings (e.g. doors, windows, vents) in structures should be located a minimum of one foot above the adjacent finished ground surface. 19.
