Directions to site: The facility is located off Old River Road at 1721 Waterway Rd., Greenville, in Pitt County, North Carolina. 6. Discharge point(s), list for all discharge points: Outfall 001 Latitude: 350 38' 04" N Longitude: 770 23' 55" W Attach a USGS Map Extract. (attached) USGS Quad map: Greenville NW, NC 7. Receiving stream or affected surface waters: Tar River a. Classification and Stream Index No.: C; NSW, 28 -(94) b. River Basin and Subbasin No.: Tar- Pamlico, 03 -03 -05 C. Describe receiving stream features and pertinent downstream uses: At the outfall, the Tar River is classified and used for secondary recreation such as boating, jet skiing and other uses involving human body contact, aquatic life including propagation and for the use of fishing to include fish consumption. (Please note that BIMS has the Outfall incorrectly classified as WS -fV which is just upstream from the discharge point and is Greenville's raw water supply intake. The receiving stream in BIMS needs to be corrected to Index 28- (94).) Page Two PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS a. Volume of wastewater: Daily average flow over the past year is approximately 0.489 MGD. b. What is the current permitted capacity: 1.2 MDG C. Actual treatment capacity of current facility (current design capacity): 1.2 MGD d. Description of existing or substantially constructed WT facilities: • Wastewater discharged results from the filter backwash and settling lagoon decant wastewater. e. Description of proposed WT facilities: None proposed. f. Possible toxic impacts to surface waters: GUC WTP poses no apparent toxic impacts to surface waters. PART III - OTHER PERTINENT INFORMATION Special monitoring or limitations (including toxicity) requests: None. 2. Compliance: The facility is in compliance with the permit. See last insppection re ort attached conducted on April 29, 2014. RECEIVEDID'ENPOW SEP 2',.3 2014 PART IV - EVALUATION AND RECOMMENDATIONS Water Quality Permitting Section Greenville Utilities Commission WTP is operating within it's permit conditions and limits.
