Dear Mr. Reading:
Division personnel have reviewed your request to modify the Daily monitoring frequency for Temperature in NPDES
NC0061123 from daily to twice per week. The Asheville Regional Office has agreed that your request has merit and that
it should be granted.
Attached please find the new modified Permit Signature page and modified A. (1.)Effluent Limitations and Monitoring
Requirements page reflecting the change for Effluent Temperature Monitoring from Daily to Twice per week. You should
replace the existing Permit Signature page and modified A. (1.)Effluent Limitations and Monitoring Requirements
page with these two modified pages. This permit correction is issued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency dated October 15, 2007 (or as subsequently amended).
Please note that this permit is not transferable except after notice to the Division. The Division may require modification
or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits
which may be required by the Division of Water Quality or permits required by the Division of Land Resources, the
Coastal Area Management Act or any other Federal or Local governmental permit that may be required. If you have any

In the event that additional facilities at this site discharge to the Little Tennessee River Basin are
constructed, the 0.006 MGD discharge into the Savannah River shall either be eliminated or
redirected to the Little Tennessee River Basin.
A. ( 3.) DECHLORINATION CONDITION
If chlorine disinfection is used, the permit holder shall employ dechlorination facilities.

Dear Mr. Poupart:
I am writing to request a modification to one of the testing parameters on our discharge permit that
requires a temperature reading frequency of 5 times/week. Due to the retirement on Dec. 8th of our
ORC, Ian Denham, we have engaged Environmental, Inc. out of Cullowhee to operate our WWTP.
Environmental continue to do our sampling and lab analysis work as well. Their technician makes two
visits/week. Our maintenance staff can take the additional temperature readings but since they are not
certified this data is not considered valid within our current permit parameters. It is the best
professional judgment of your regional staff that this modification does not in any way compromise the
performance standards by which our permit was written.
This modification request is based on the following considerations: The Mountain is a small non-profit
organization. There would be an additional financial hardship if we must bring in an outside firm three
additional visits/week. We are located on top of Little Scaly Mountain in a very remote corner of Macon
County. Our facility is located at the end of a long primitive service road. Our plant uses two sand filters
for primary treatment. There are no mechanical systems that can fail, that would require immediate
response. Finally, this plant has a very small and intermittent outflow.
We would greatly appreciate a favorable consideration for this request. We will be submitting an
application for permit renewal before March 4`h. We would like to request this testing modification be
written into our new permit parameters if possible. Please note that both Gary Francies from the Lab
Certification Section and Mark Teague, owner of Environmental, Inc. support this request. Thank you.

I forgot to mention — please copy me on any communication that is sent to The Mtn. Retreat modifying their
temperature monitoring. Once I receive that I can clean up things for them on the laboratory certification side. You may
