Permit No. NC0044962
North Stokes High School
Stokes County
Dear Mr. Goins:
The Division of Water Quality (DWQ) has found typographical errors in the effluent
limitations page (A. (1.)) of the NPDES permit issued to you on January 25, 2002. Please
find enclosed the revised permit page. The revised page should be inserted into your
permit. The old effluent limitations page may then be discarded. All other terms and
conditions contained in the original permit remain unchanged and in full effect. This
permit modification is issued under the requirements of North Carolina General Statutes
143-215.1 and the Memorandum of Agreement between North Carolina and the U. S.
Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit
modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must
be a written petition conforming to Chapter 150B of the North Carolina General Statutes,
filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and
binding.
If you have any questions concerning this permit modification, please contact Christie
Jackson at (919) 733-5083, extension 538.
Sincerely,
regory J. Thorpe, Ph.D.

Christie,
In reviewing the new permit issued to this facility dated January 25, I noticed that the sample type was
composite for whole effluent toxicity. The previous permit had been modified to specify the sample type as
grab. When I went back to look at the draft permit that I reviewed in the fall, the sample type was indeed
composite; so I missed this when I reviewed the draft.
The problem is that I'm pretty sure the facility will not be able to collect a composite sample. Is there any way
we can address this other than a permit modification and, if not, is a permit modification even feasible at this
point given personnel shortages?
