Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007
(or as subsequently amended).
Please note that the receiving stream is listed as impaired for benthos on the North Carolina 2016
303(d) Impaired Waters List. Addressing impaired waters is a high priority with the Division, and
instream data will continue to be evaluated. If there is noncompliance with permitted effluent
limits and stream impairment can be attributed to your facility, then mitigative measures may be
required.
No changes were made to the draft permit sent to you on October 16, 2018.
The final permit maintains the following significant changes identified in the letter sent on
October 16, 2018:
• Federal regulations require electronic submittal of all discharge monitoring reports
(DMRs) and program reports. The requirement to continue reporting discharge monitoring
data electronically using the NC DWR's Electronic Discharge Monitoring Report (eDMR)
internet application has been added to your NPDES permit [See Special Condition A. (6.)].
• Parameter codes have been added to the Effluent Limitations and Monitoring Requirements
[ See A. (1.)].
• Regulatory citations have been added to the permit.

• Special Condition A. (3.) Reporting of Operation has been revised and renamed as Special
Condition A. (3.) Reactivation of Discharge and includes additional language to ensure the
proper maintenance and operation of the facility.

unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect
the legal requirements to obtain other permits which may be required by the Division of Water
Resources or any other Federal, State, or Local governmental permits that may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
SKF USA Inc.
is hereby authorized to discharge wastewater from a facility located at
4328 South York Street
Gastonia
Gaston County
to receiving waters designated as Crowders Creek in the Catawba River Basin in accordance with
the discharge limitations, monitoring requirements, and other conditions set forth in Parts I, II, III,
and IV hereof.
This permit shall become effective January 1, 2019.
This permit and the authorization to discharge shall expire at midnight on August 31, 2022.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises
under the permit conditions, requirements, terms, and provisions included herein.
SKF USA Inc. is hereby authorized to:
1. Continue to operate and maintain an existing 0.0072 MGD groundwater treatment facility
consisting of the following components:

In accordance with Part II Section C-1, the ORC or back-up ORC of the facility shall visit the facility
at least weekly.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

A. (2.) ACUTE TOXICITY PASS/FAIL PERMIT LIMIT (QRTRLY)
[15A NCAC 02B .0200 et seq.]
The permittee shall conduct acute toxicity tests on a quarterly basis using protocols defined in the
North Carolina Procedure Document entitled "Pass/Fail Methodology For Determining Acute
Toxicity In A Single Effluent Concentration" (Revised December 2010 or subsequent versions). The
monitoring shall be performed as a Fathead Minnow (Pimephales promelas) 24-hour static test.
The effluent concentration at which there may be at no time significant acute mortality is 90%
(defined as treatment two in the procedure document). The tests will be performed during the
months of February, May, August and November. These months signify the first month of each
three-month toxicity testing quarter assigned to the facility. Effluent sampling for this testing
must be obtained during representative effluent discharge and shall be performed at the NPDES
permitted final effluent discharge below all treatment processes.
Should any single quarterly monitoring indicate a failure to meet specified limits, then monthly
monitoring will begin immediately until such time that a single test is passed. Upon passing, this
monthly test requirement will revert to quarterly in the months specified above.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGE6C. Additionally, DWR Form AT-2 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Water Sciences Section no later than
30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical
measurements performed in association with the toxicity tests, as well as all dose/response data.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is
employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring
is required, the permittee will complete the information located at the top of the aquatic toxicity
(AT) test form indicating the facility name, permit number, pipe number, county, and the
month/year of the report with the notation of "No Flow" in the comment area of the form. The
report shall be submitted to the Water Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately until such time that a single test is passed. Upon
passing, this monthly test requirement will revert to quarterly in the months specified above.
Assessment of toxicity compliance is based on the toxicity testing quarter, which is the three-
month time interval that beginson the first day of the month in which toxicity testing is required
by this permit and continues until the final day of the third month.

Should any test data from either these monitoring requirements or tests performed by the North
Carolina Division of Water Resources indicate potential impacts to the receiving stream, this
permit may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival and appropriate environmental controls, shall constitute an invalid test
and will require immediate follow-up testing to be completed no later than the last day of the
month following the month of the initial monitoring.
A. (3.) REACTIVATION OF DISCHARGE
[G.S. 143-215.1(a)]
The Permittee and/or Permittee representative shall contact the Mooresville Regional Office Water
Quality Regional Operations staff (phone: (704) 663-1699, during normal business hours), at least
90 days prior to resuming operation and discharge, to request a Compliance Inspection. Contact
information shall include the Permittee's (1) contact person name and phone number, (2) NPDES
permit number, (3) planned date and time of resumption of operation, and (4) expected date
discharge is to resume. Additional information may be requested by Regional staff. Regional staff
will perform a Compliance Inspection and the Permittee will be required to confirm (1) treatment
equipment has been serviced and is functioning properly, (2) recovery well pumps have been
serviced and are functioning adequately and (3) the outfall is accessible and adequately maintained.
In addition, a list of chemicals used or stored at the facility shall be provided to the Regional
inspector. Operation and discharge from the facility can commence provided the inspection report
shows the facility is compliant with the inspection items and the items listed above. Upon start up,
any previous monitoring/reporting waivers are no longer applicable.

A. (4.) VOLATILE ORGANIC COMPOUNDS AND 1,4-DIOXANE SPECIAL MONITORING

Within 30 days of commencing discharge, the Permittee shall perform one (1) Pollutant Scan for all
parameters listed below at their designated sample locations. Analytical methods shall be in
accordance with 40 CFR Part 136 unless otherwise specified and shall be sufficiently sensitive to
determine whether parameters are present in concentrations greater than applicable standards and
criteria.

[G.S. 143-215.1]
Pursuant to N.C. General Statutes Section 143-215.1 and the implementing rules found in the
North Carolina Administrative Code at 15A NCAC 2H.0112 (b) (1) and 2H.0114 (a) and Part II
sections B-12 and B-13 of this permit, the Director of DWR may reopen this permit to require
additional monitoring or effluent limitations for the discharge based on the sample results
required by Special Condition A. (4.) Volatile Organic Compounds and 1,4-Dioxane Special
Monitoring.

A. (6.) ELECTRONIC REPORTING OF MONITORING REPORTS
[G.S. 143-215.1(b)]
Federal regulations require electronic submittal of all discharge monitoring reports (DMRs) and
program reports. The final NPDES Electronic Reporting Rule was adopted and became effective on
December 21, 2015.
NOTE: This special condition supplements or supersedes the following sections within Part II of this
permit (Standard Conditions for NPDES Permits):

Upon reactivation of the facility, the permittee shall report discharge monitoring data
electronically using the NC DWR's Electronic Discharge Monitoring Report (eDMR) Internet
application.
Monitoring results obtained during the previous month(s) shall be summarized for each month
and submitted electronically using eDMR. The eDMR system allows permitted facilities to enter
monitoring data and submit DMRs electronically using the internet. Until such time that the
state's eDMR application is compliant with EPA's Cross -Media Electronic Reporting Regulation
(CROMERR), permittees will be required to submit all discharge monitoring data to the state
electronically using eDMR and will be required to complete the eDMR submission by printing,
signing, and submitting one signed original and a copy of the computer printed eDMR to the
following address:

If a permittee is unable to use the eDMR system due to a demonstrated hardship or due to the
facility being physically located in an area where less than 10 percent of the households have
broadband access, then a temporary waiver from the NPDES electronic reporting requirements
may be granted and discharge monitoring data may be submitted on paper DMR forms (MR 1,
1.1, 2, 3) or alternative forms approved by the Director. Duplicate signed copies shall be
submitted to the mailing address above. See "How to Request a Waiver from Electronic
Reporting" section below.
Regardless of the submission method, the first DMR is due on the last day of the month following
the issuance of the permit or in the case of a new facility, on the last day of the month following
the commencement of discharge.
Starting on December 21, 2020, the permittee must electronically report the following
compliance monitoring data and reports, when applicable:
• Sewer Overflow/Bypass Event Reports;
• Pretreatment Program Annual Reports; and
• Clean Water Act (CWA) Section 316(b) Annual Reports.

The permittee may seek an electronic reporting waiver from the Division (see "How to Request
a Waiver from Electronic Reporting" section below).
2. Electronic Submissions
In accordance with 40 CFR 122.41(1)(9), the permittee must identify the initial recipient at the
time of each electronic submission. The permittee should use the EPA's website resources to
identify the initial recipient for the electronic submission.
Initial recipient of electronic NPDES information from NPDES-regulated facilities means the
entity (EPA or the state authorized by EPA to implement the NPDES program) that is the
designated entity for receiving electronic NPDES data [see 40 CFR 127.2(b)].
EPA plans to establish a website that will also link to the appropriate electronic reporting tool
for each type of electronic submission and for each state. Instructions on how to access and
use the appropriate electronic reporting tool will be available as well. Information on EPA's

Electronic submissions must start by the dates listed in the "Reporting Requirements" section
above.
3. How to Request a Waiver from Electronic Reporting
The permittee may seek a temporary electronic reporting waiver from the Division. To obtain
an electronic reporting waiver, a permittee must first submit an electronic reporting waiver
request to the Division. Requests for temporary electronic reporting waivers must be submitted
in writing to the Division for written approval at least sixty (60) days prior to the date the facility
would be required under this permit to begin submitting monitoring data and reports. The
duration of a temporary waiver shall not exceed 5 years and shall thereupon expire. At such
time, monitoring data and reports shall be submitted electronically to the Division unless the
permittee re -applies for and is granted a new temporary electronic reporting waiver by the
Division. Approved electronic reporting waivers are not transferrable. Only permittees with an
approved reporting waiver request may submit monitoring data and reports on paper to the
Division for the period that the approved reporting waiver request is effective.

Briefly describe the proposed permitting action and facility background: The permit was originally issued
to CR Industries for the treatment of domestic and industrial waste. The permit has been modified
extensively since it was issued in 1996. In 1998 they added a groundwater treatment system and began
discharging the treated groundwater through the same outfall as the industrial/domestic treatment plant. In

1999 they closed the facility and ceased the domestic/industrial discharge. They eventually sold the facility
and the new owner connected the domestic/industrial portion of the effluent to the City of Gastonia WWTP.
SKF USA retained the groundwater treatment system and the responsibility for its operation and
maintenance. The groundwater remediation system was installed to remediate/extract trichloroethene,
tetrachloroethene, and cis-1,2dichloroethene from the groundwater.
Initially, a Chronic toxicity test at 0.33% concentration was required for the GWR discharge. But in the
2002 permit renewal the permitted flow was decreased resulting in the toxicity test requirement being
changed to Acute to comply with Division policy. The policy stated that when the toxicity test
concentration (defined by the IWC) is less than 0.25% then a Chronic test is not applicable and an Acute
toxicity test at 90% must be applied.
The last reported discharge from this GWR system was in March 2008. In February 2010, the permittee
requested and was granted a waiver to discontinue monitoring and submittal of DMR while no discharge
occurred. The treatment system is currently deemed inactive, and the region has confirmed that
subsequent piping had been removed to disengage the treatment system from the outfall. All fees and
other permit conditions still apply.
The groundwater treatment system consists of 2 collection wells which discharge to a 500 gallon surge
tank. A transfer pump conveys the liquid from the surge tank through a bag filter and then to a 5 tray air
stripper. As the flow leaves the air stripper collection sump, a flow meter/recorder measures the flow. The
effluent flow continues through a buried gravity line and discharges through the permitted Outfall 001
into Crowders Creek. Crowders Creek flows into South Carolina and is a tributary to Lake Wylie.
Hart & Hickman, PC, representative/operator for this GWR system has submitted a permit renewal as
they believe they will be required to restart this system. The DWM incident number for this facility is
NCD006556963.
In 2010, the Division received and reviewed a request for a waiver of monthly reporting requirements. As
the facility had not been producing any wastewater discharge and the remediation system had been
inactive since 2008, the request was granted. The site remains inactive. The Permittee shall contact and
notify the Mooresville Regional Office within 30 days of site reactivation.
A special condition has been added to the permit requiring the Permittee to conduct an influent and
effluent VOC scan as well as an effluent 1,4-dioxane scan within 30 days of site reactivation. As
chlorinated solvents were present at the site, 1,4-dioxane may also be present in the groundwater. The
addition of these scans acts as a reasonable precaution. A second, complementary special condition was
added to the permit allowing the Division to reopen the permit to address pollutants of concern in the case
the required scans turn up results with reasonable potential for harm.
Note: The draft permit was addressed to the incorrect person but was received and reviewed by the
Permittee. The fmal cover letter has been adjusted and addressed to Erika Mau, the current authorized
SKF USA, Inc. representative.

5. Compliance Summary

Summarize the results from the most recent compliance inspection: The last facility inspection conducted
in March 2017 reported that the facility was neither in nor out of compliance. According to the report,
Mooresville Regional Office staff were unable to gain access to the two onsite recovery wells, as they
were locked and a key was unavailable at the time. The inspection summary explains that the system was
shut down in 2008 and is in an inoperative state. It was recommended that, should the system be
reactivated, the system discharge be connected to a nearby storm drain. The MRO would need to be
present at system startup in this case.
6. Water Quality -Based Effluent Limitations (WQBELs)
Dilution and Mixing Zones
In accordance with 15A NCAC 2B.0206, the following streamflows are used for dilution considerations
for development of WQBELs: 1 Q 10 streamflow (acute Aquatic Life); 7Q10 streamflow (chronic Aquatic
Life; non -carcinogen HH); 30Q2 streamflow (aesthetics); annual average flow (carcinogen, HH).

Oxygen -Consuming Waste Limitations
Limitations for oxygen -consuming waste (e.g., BOD) are generally based on water quality modeling to
ensure protection of the instream dissolved oxygen (DO) water quality standard. Secondary TBEL limits
(e.g., BOD= 30 mg/1 for Municipals) may be appropriate if deemed more stringent based on dilution and
model results.

Ammonia and Total Residual Chlorine Limitations
Limitations for ammonia are based on protection of aquatic life utilizing an ammonia chronic criterion of
1.0 mg/1 (summer) and 1.8 mg/1 (winter). Acute ammonia limits are derived from chronic criteria,
utilizing a multiplication factor of 3 for Municipals and a multiplication factor of 5 for Non -Municipals.
Limitations for Total Residual Chlorine (TRC) are based on the NC water quality standard for protection
of aquatic life (17 ug/1) and capped at 28 ug/1 (acute impacts). Due to analytical issues, all TRC values
reported below 50 ug/1 are considered compliant with their permit limit.
Describe any proposed changes to ammonia and/or TRC limits for this permit renewal: N/A
Reasonable Potential Analysis (RPA) for Toxicants
If applicable, conduct RPA analysis and complete information below.
As the facility has been in an inactive state and has reported no flow, no RPA was conducted.
Toxicity Testing Limitations
Permit limits and monitoring requirements for Whole Effluent Toxicity (WET) have been established in
accordance with Division guidance (per WET Memo, 8/2/1999). Per WET guidance, all NPDES permits
issued to Major facilities or any facility discharging "complex" wastewater (contains anything other than
domestic waste) will contain appropriate WET limits and monitoring requirements, with several
exceptions. The State has received prior EPA approval to use an Alternative WET Test Procedure in
NPDES permits, using single concentration screening tests, with multiple dilution follow-up upon a test
failure.
Describe proposed toxicity test requirement: This is a Minor Industrial facility. Should the facility
reactivate, an acute WET limit at 90% effluent will continue on a quarterly frequency.
Mercury Statewide TMDL Evaluation
There is a statewide TMDL for mercury approved by EPA in 2012. The TMDL target was to comply
with EPA's mercury fish tissue criteria (0.3 mg/kg) for human health protection. The TMDL established a
wasteload allocation for point sources of 37 kg/year (81 lb/year), and is applicable to municipals and
industrial facilities with known mercury discharges. Given the small contribution of mercury from point
sources (-2% of total load), the TMDL emphasizes mercury minimization plans (MMPs) for point source
control. Municipal facilities > 2 MGD and discharging quantifiable levels of mercury (>1 ng/1) will
receive an MMP requirement. Industrials are evaluated on a case -by -case basis, depending if mercury is a
pollutant of concern. Effluent limits may also be added if annual average effluent concentrations exceed
the WQBEL value (based on the NC WQS of 12 ng/1) and/or if any individual value exceeds a TBEL
value of 47 ng/1.
Describe proposed permit actions based on mercury evaluation: The facility is not known to discharge
mercury.

7. Antidegradation Review (New/Expanding Discharge):
The objective of an antidegradation review is to ensure that a new or increased pollutant loading will not
degrade water quality. Permitting actions for new or expanding discharges require an antidegradation
review in accordance with 15A NCAC 2B.0201. Each applicant for a new/expanding NPDES permit
must document an effort to consider non -discharge alternatives per 15A NCAC 2H.0105( c)(2). In all
cases, existing instream water uses and the level of water quality necessary to protect the existing use is
maintained and protected.

8. Antibacksliding Review:
Sections 402(o)(2) and 303(d)(4) of the CWA and federal regulations at 40 CFR 122.44(1) prohibit
backsliding of effluent limitations in NPDES permits. These provisions require effluent limitations in a
reissued permit to be as stringent as those in the previous permit, with some exceptions where limitations
may be relaxed (e.g., based on new information, increases in production may warrant less stringent TBEL
limits, or WQBELs may be less stringent based on updated RPA or dilution).

9. Monitoring Requirements
Monitoring frequencies for NPDES permitting are established in accordance with the following
regulations and guidance: 1) State Regulation for Surface Water Monitoring, 15A NCAC 2B.0500; 2)
NPDES Guidance, Monitoring Frequency for Toxic Substances (7/15/2010 Memo); 3) NPDES Guidance,
Reduced Monitoring Frequencies for Facilities with Superior Compliance (10/22/2012 Memo); 4) Best
Professional Judgement (BPJ). Per US EPA (Interim Guidance, 1996), monitoring requirements are not
considered effluent limitations under Section 402(o) of the Clean Water Act, and therefore anti -
backsliding prohibitions would not be triggered by reductions in monitoring frequencies.

10. Electronic Reporting Requirements
The US EPA NPDES Electronic Reporting Rule was finalized on December 21, 2015. Effective
December 21, 2016, NPDES regulated facilities are required to submit Discharge Monitoring Reports
(DMRs) electronically. Effective December 21, 2020, NPDES regulated facilities will be required to
submit additional NPDES reports electronically. This permit contains the requirements for electronic
reporting, consistent with Federal requirements.

Staff from the MRO visited the former SKF facility. We were not able to gain access to the two onsite
recovery wells as the well out protective covers were locked and no key was available to open the
locks.
This system was shut down in 2008 and the system has be placed into an inoperative state. Except for
the removal of system plumbling, the primary components of the system are present. As the system
has been inactive for almost ten years, a good deal of work would be required to place the system back
into an operational state should the permittee ever wish to reactivate the system.
The permittee applied for a permit renewal in June 2015. The renewal application is currently listed as
in Review status with Central Ofice Permitting. Due to permitting back log,renewal process is delayed.

Comment: The system has been shut down since 2008. Treatment system piping has been removed
and therefore the system has been made to be inoperable. There are currently no plans to
restart the system in the near future.
During the inspection. the out fall pipe was not found. There appears to be a storm drain
pipe that is located behind the guard shack where the treament system is housed. The pipe
was damaged during re -paving operations of a couple of years ago.
It would be recommended that should the system be reactived. system discharge be
connected to a nearby storm drain. Currently, the outfall appears to be connected to an
intermittant stream channel. No discharge was noted to be occurina at the time of the site
visit.
Should the system be brought back online. the MRO would need to be present at system
startup.

I do not see any concerns that MRO would have regarding the renewal of this permit.
Is there a plan to re -activate the system? If so, the permittee would need to bring the system up to
current operational standards. Based on the observations of the treatment system in 2017, a good deal
of work would need to be done to the system to make it operational again. The Air Stripper would be
required to be serviced and the surge tank would need to be leak checked as there was evidence of
mice/rats in the system containment area during the 2017 site visit.
Also, the recovery well pumps would most likely need to be removed and replaced along with pressure
gauges', electrical systems and hoses.
The list of constituents to be monitored appear to be quite comprehensive, the MRO would request that
a list of chemicals being stored at the facility be provided by the permittee. This would be
documentation above the constituents being addressed by the on -site treatment system. This list of
chemicals would allow for system modifications to be made for the treatment of the additional
chemicals that may be introduced to the GW treatment system.
Another issue would be outfall maintenance. During the 2017 site visit, we were not able to identify the
outfall location. There was a lot of erosion in the drainage ditch.
The GW monitor wells would need to be verified and GPS coordinates should be provided for the well
locations. If I remember correctly, the wells had tags on them. However, I am not sure if the tags
provided coordinates. We would also want to the wells to be re -developed as they have sat since 2010.
Well repairs would need to be made if deemed necessary.

Dear Cory,
A little while back I sent a draft copy of the NPDES Permit NC0004260 for the Former CR Industries site for review by
MRO. I received some comments/concerns and sat with my supervisor to address them. We feel we have done so and
would like your group to confirm that the majority of their concerns were addressed within reason so we can officially
submit the permit to public notice.
To summarize the situation, the remediation site has been inoperative since 2010 and, according to MRO, has not been
maintained very well. I received a list of components with maintenance issues including the facility's air stripper,
recovery well pumps, and even their outfall. Additionally MRO asked for a list of chemicals being stored at the facility. To
address these concerns, we incorporated a special condition A. (3.) Reactivation of Discharge, which requires the
Permittee to contact MRO 90 days prior to resuming operation in order to request and have a compliance inspection.
We lay out the information that the Permittee must present the region, explain that the regional office will look for the
components that were listed in MROs comments to make sure they are functioning adequately, and that the Permittee
must present the region with a list of stored and used chemicals. Only if they pass the region's inspection may they
commence discharge.

Dear Ms. Thedford:
The enclosed permit renewal application is being submitted by SKF USA Inc. for NPDES
permit NCG00426O. This permit is for a ground water remediation pump and treat
system at the former CR Industries site in Gastonia, NC. Similar to the prior renewal the
ground water remediation system has not been used since March 2008. While there are
no immediate plans to restart the system, we wish to renew the permit to allow use
should the need arise. This submittal includes one original and two copies of the
following information:
• This cover letter requesting renewal of the permit and documenting changes at the
facility since issuance of the last permit.
o The completed application form, signed by the permittee.
• This narrative description of the sludge management plan for the facility: Because
this system uses air stripping as the treatment mechanism to remove volatile
contaminants, the system does not produce wastewater treatment sludge.
