4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
Stream restoration and Stabilization are planned for the stream running through the YMCA of Catawba property.
Buildings were built in 2005, 2008 and 2009. All area on the property is developed.
Project property is surrounded by 1/4 acre developed home lots.
Stream runs between two buildings with approximately 40 ft in most places between road access on one side and side walk on other side.

The project purposed is to stabilize eroding banks and provide a stable channel pattern, profile and dimensions.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The 280ft upstream of the culvert crossing of 1st St. NW will be comprised of a bankfull bench, coir matting, seeding with native vegetation and planting a 25ft buffer on right side of the
channel (facing downstream). Spot areas of bank erosion caused by overland concentrated flows from the parking area, will be stabilized with toe rock and rock stabilization.
The 680 ft below the culvert will be graded to a stable pattern, profile and dimension. Two J-Hooks will be installed.
I anticipate an excavator with a hydraulic thumb (possibly two), a bob cat and a dump truck will be used to construct the project.

The existing conditions of the bed and bank material were taken into consideration during the planning and design of this project. Areas where the
channel was fundamentally stable only a bankfull bench installation has been planned.
In areas where the channel has eroding banks and profile aggregation, a restoration has been planned to implement correct pattern, profile and
dimensions.
All areas worked on will have a minimum 10 ft buffer on each side planted with native vegetation.
lb. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Project will only disturb what can be stabilized daily.
Upper section should only take 1 to 3 days to construct and stabilize.
Lower section (restoration) should be completed in 7 to 10 working days. Anticipated weather will be monitored daily.
Design engineer or designated technical staff will be onsite daily to ensure areas are stabilized at the end of each day.
