1. The annual fee of $360 for permit WQ0003380 is overdue as of July 3, 2022. This payment is
for the period beginning on July 1, 2022, through June 30, 2023. This payment was due on July
3, 2022, as listed in Invoice #: 2022PR005512. This permit renewal will not be issued until the
annual fee is paid pursuant to 15A NCAC 02T .0120(c).
