Dear Mr. Houck:
Ron Boone and Seren Homer, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water
Resources (DWR), conducted a compliance evaluation inspection (CEI) of the Little Creek Diner's
wastewater treatment and disposal system on July 25t", 2022. Inspection findings are detailed in the
attached water compliance inspection report.
The only discrepancy Mr. Boone noted is that our database currently shows the owner of the facility as
Arzell Tuttle, and the name of the facility as Tuttle's Creekside Restaurant. This must be updated.
Please fill out the attached name/ownership change form completely and in accordance with all
instructions on the form. Mail the completed form, along with all required documentation, to our
Raleigh Central Office at the address indicated on the form.

Ron Boone and Seren Homer, of the Winston-Salem Regional Office (WSRO) of the NC Division of
Water Resources (DWR), conducted a compliance evaluation inspection (CEI) of the wastewater
treatment and disposal system at the Little Creek Diner in Germanton, NC on July 25th, 2022.
Inspection findings are
summarized below.
Grease trap and septic tank pumping and maintenance records were reviewed. Records indicate the
tanks are all being maintained in top condition.
The chlorine tablets being used at the facility are of the correct type. The chlorinator appears to be in
good condition.
The discharge to the stream was inspected. No issues were noted. There was only a drip coming out
the end of the pipe.
Analytical records were reviewed. The treatment system appears to be working well as all results
were well below permit limits.
The system appears to be in very good condition. No conditions indicating problems or malfunctions
were noted.
The only discrenancy noted during the inspection is that the owner name needs to be changed on
the permit. A name/ownership change form will be attached to this inspection report. Please fill it out
and send it to the address indicated on the form. Please follow all directions on the form.
