Ron Boone and Seren Homer, of the Winston-Salem Regional Office (WSRO) of the NC Division of
Water Resources (DWR), conducted a compliance evaluation inspection (CEI) of the wastewater
treatment and disposal system at the address covered by the subject permit on 08/11/2022.
It was apparent that no one had lived at the address for quite some time. There are several separate
residential structures on the property. The chlorinator was identified but the discharge pipe could not
be located. No other components could be located.
Upon return to the office, an Interent search revealed the property was sold by Karen Harris to
Mountain Valley Pipeline (MVP) LLC in 5/2020. Mr. Boone contacted MVP and spoke with Wallette
Shealey. Mr. Boone briefed Ms. Shealey on the existence of the permit. Ms. Shealey expressed that
MVP would like to maintain the permit in good standing in case they sell the property in the future. Mr.
Boone informed Ms. Shealey that MVP would need to pay all due annual permit fees as well as submit
a Name/Ownership change form to the Division to thave the permit placed in their name. Mr. Boone
requested the due invoices from Central office and sent them to Ms. Shealey for payment. Mr. Boone
also sent her a Name/Ownership change form.
