HODGKINS – TOWN MANAGER TOWN OF FARMVILLE POST OFFICE BOX 86 FARMVILLE, NORTH CAROLINA 27828 Subject: Permit No. WQ0008778 Town of Farmville Class B Residuals Program Attachment B Certification Pitt County Dear Mr. Hodgkins: In accordance with your permit major modification request received April 21, 2022, and subsequent additional information received July 28, 2022, we are forwarding herewith a modified Attachment B certified September 19, 2022, to the Town of Farmville for the subject residuals management program. Modifications to the subject Attachment are as follows: Addition of Field NC-PI-06-01 in Pitt County, with a total of 54.2 acres. The modified Attachment B shall be effective from the date of certification until January 31, 2028 (i.e., expiration date of the current permit), shall replace Attachment B of Permit No. WQ0008778 issued November 15, 2021, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above-referenced permit. Please replace Attachment B of Permit No. WQ0008778 that was issued November 15, 2021 with the modified Attachment B. Please note that the Permittee shall submit a renewal application no later than August 4, 2027. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee’s responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding. Mr. David P.
