Acting Secretary Subject: Permit No. WQ0033325 Additional Information Request Tobermory Well Wastewater Irrigation System Bladen County Division of Water Resources' Central and Regional staff has reviewed the application package received September 15, 2016. However, additional information is required before the review may be completed. Please address the items on the attached page no later than the close of business on April 5, 2017. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the appropriate items outstanding in Sections A or B, or failure to provide the additional information on or before the above requested date, may result in your application being returned as incomplete. Please reference the subject application nurnber when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with three copies submitted to my attention at the address below. Non -Discharge Wastewater System Waiver: The setback waiver signed by Earl and Denise Goings on January 11, 2017 indicated that they were the owners of the property located at 8129 Chickenfoot Road. That property is actually owned by the Bladen County Water District. The correct address for the adjacent property owned by Earl and Denise Goings has been entered into the waiver form we have filled out and attached herewith for your use. Please have the attached waiver form completed, signed, sealed and recorded in the Bladen County Register of Deeds, and copied to us. B. Non -Discharge Wastewater System Waiver: The setback waiver signed by Mary Beth Edge on January 5, 2017 indicated that she was the owner of the property located at 8129 Chickenfoot Road. That property is actually owned by the Bladen County Water District. The correct address for the adjacent property owned by Mary. Beth Edge has been entered into the waiver form we have filled out and attached herewith for your use. The attached waiver form has also had the setback adjusted from 150 feet to.5 feet. . Please have the attached waiver form completed, signed, sealed and recorded in the Bladen County Register of Deeds, and copied to us.
