Dear Mr. McDonald:
On September 24. 2014, the, Division of Water Resources (Division) received your application
requesting a Individual Water Quality Certification from the Division for the subject project. The
Division has determined that your�application is incomplete,and, cannot be processed. The
application is on -hold until -all of the following information is received:

A review of the Standard Permit Form indicates (EN 4345) this application is for 4.021 acres
of wetland impacts. Pursuant to 143- 215.3D (e), a $570 permitting fee is required to
process this application. We understand that you will be providing that fee electronically
For your reference our tax ID number is 56- 6000372.
2. Need more-copies of the application
Pursuant to 15A NCAC 02H .0502 and G.S. 143- 215.3D(e), the Division requires four (4)
complete application packages, including the appropriate fee, to begin the proper
consideration of an application. We received one (1) copy of the application, however we
still need 3 copies of the application. Please submit 3 additional copies of the application
Pursuant to Title 15A NCAC 02H .502(e), the applicant shall furnish all of the above requested
iniformation for the proper consideration of the application. If all of the requested information'is
not received in writing at the address below within 30 calendar days of receipt of this letter, the
Division will be unable to process the application and it will be returned. The return of this project
will necessitate reapplication to the Division for approval, including a complete application
package and the appropriate fee.
