Hello all,
Our previous request (7/11/22) was rescinded (7/18/22) to include additional revisions to allow for one in-field mod to
address multiple sites.
It was discussed at an agency meeting early June that we had some issues with pipes, headwall and/or scour
pads going outside of our permitted sections and into the mechanized clear zones. The attached Excel
spreadsheet includes locations where we need additional impacts added. Garcy asked for these to be added to
the pending request we have for structure 3410 at Permit Site 38A (see “20 sq ft mod” email ) which DCM has
approved.
Included in the attached drawings are the proposed overhead lines at Slocum Creek for the DMS structures. Per
Division 2, Stephen said we could include it with the current mod we are working on. There are no buffer zone
impacts per Garcy and Stephen.
Permit Site 2: increase in excavation of 0.03 ac & decrease in Mech. Clearing of 0.03 ac due to the fill sloping
being pushed out due to addition of guardrail around overhead sign.
Permit Site 3: Increase in Fill in wetlands of 0.08 ac, Increase in Excavation of 0.005 ac & Decrease in Mech
Clearing of 0.03 ac due to the fill sloping being pushed out due to addition of guardrail around overhead sign.
For sites 2 and 3, the changes are minor and cannot be easily seen on the permit drawing sheets, therefore
drawings at these sites have not changed.

Permit Site 39: Increase in fill in wetlands of 0.01 ac due to error/ missing sliver of fill & Increase in Mech
Clearing of 0.02 ac due to trees in wetland needing to be removed. Revised permit drawings (sheets #133 & 135
) that show the revision for site 39 are attached.
Please let me know if you need any additional information and if we need to provide mitigation for the revised impacts.
