located at 200 Ridgefield. Court, Asheville, NC 28806. Staff from the Asheville Regional Office has confirmed
-that this well. was never installed or is no longer being used for injection, and a permit.is no longer required.
Therefore, in accordance with your request, Underground Injection Control (UIC) Permit WI0100137 is
rescinded, effective immediately.
If in the future, you. wish to operate a Type 5QW geothermal heat pump injection system, you must first'submit
a Notification of Intent to Construct'a 5QW Geothermal Well, Operating a geothermal heat pump injection
system without a valid permit may result in the assessment of civil penalties and/or the use of other enforcement
mechanisms available to the state.

Dear Mr. Archerd:
. On December.16, 2010, I made a site visit to 200 Ridgefield Court and observed Steven
DeWeese, an employee of GeoTherm Loop Services, Inc., in the process of completing the abandonment
of a well at the property referenced above. Mr. DeWeese said that he was currently completing the
abandonment of the second and final well that had been constructed at this site. Mr. DeWeese further
explained that GeoTherm Loop Services, inc. would'not be drilling any other wells on the property.
I spoke with you on January 19, 2011 and asked if you planned to pursue the installation of
additional injections wells at 200 Ridgefield Court. You explained that you were no longer interested in
installing any injection wells, as indicated in the GPU/UIC 5QW Notification of Intent Form that you
submitted and was received by the Aquifer Protection Section on December 9, 2010. I requested that you
send me a letter stating that you wanted permit number WI0100137 rescinded and you agreed to draft
the letter, sign it, and send me an electronic copy. I contacted you on January 26, 2011 to inform you
that I had not received this letter and you stated that you had sent it just after our conversation earlier in
the month. You agreed to send me the letter when you returned to your office on January 28, 2011.
Please consider this my third request to obtain a letter from you concerning the rescission of
permit number WI0100137; you may send the letter via mail, e-mail or fax. If you have any questions

On 12/09/2010, the Aquifer" Protection Section (APS) received notification of your intent to construct a closed -loop water -only
geothermal injection well system for the operation -of a ground -source heat pump located at the address referenced above. An
individual permit is not required for the construction and operation of this type of geothermal injection well system as long as the
following conditions are met:
1. The injection well system contains only potable water,
2. The injection well system is constructed in accordance with well construction standards specified in North
Carolina Administrative Code Title 15A Section.2C Subchapter .0213, and
3. The required notification form and associated maps have been completely and accurately submitted.
Failure to comply with all of these conditions constitutes a violation of the North Carolina Well Construction Act and North Carolina
Administrative Code Title 15A Section 2C Subchapter .0211(u)(2). Additionally, you should contact the Buncombe County Health
Department as they may have additional requirements for this type of system. Noncompliance with applicable state, county, or
municipal rules and regulations may result in the assessment of civil,penalties.

(2) The Site Map must show the subject property in relation to the surrounding area by using at least two fixed
