ENGINEER'S NOTES: 1) THE LOCATION AND ALIGNMENT OF ALL IMPROVEMENTS SHOWN ARE APPROXIMATE. THE FINAL LOCATION AND ALIGNMENT SHALL BE DETERMINED DURING CONSTRUCTION USING BEST PROFESSIONAL JUDGMENT AND APPROVED BY THE PROJECT ENGINEER. 2) MATERIALS LISTS ARE PROVIDED AS AN APPROXIMATION ONLY. NCDEQ "EROSION AND SEDIMENT CONTROL PLANNING AND DESIGN MANUAL, 2013. 5) THE CONTRACTOR SHALL CALL THE "CALL BEFORE YOU DIG" TOLL FREE NUMBER TO MAKE SURE THAT ALL UTILITIES ARE LOCATED AND MARKED. ANY DAMAGE TO EXISTING UTILITIES IS THE RESPONSIBILITY OF THE CONTRACTOR. 6) THE CONTRACTOR SHALL CONDUCT PRE- AND POST- CONSTRUCTION VIDEO INSPECTION OF UNDERGROUND UTILITIES TO ENSURE NO PROJECT RELATED DAMAGE. 7) ANY DAMAGE RESULTING FROM ACTIONS OF THE CONTRACTOR TO STRUCTURES OR GROUNDS ON SITE WILL BE REPAIRED OR REPLACED AT THE EXPENSE OF THE CONTRACTOR, TO A QUALITY MEETING OR EXCEEDING THEIR PREVIOUS STANDARDS. 8) THE CONTRACTOR SHALL PROVIDE FOR THE STRUCTURAL PROTECTION OF EXISTING SEWER INFRASTRUCTURE AND PROVIDE PLANS FOR ANY NECESSARY STRUCTURAL REINFORCEMENT MEASURES. 9) THE CONTRACTOR IS TO ENSURE PUBLIC SAFETY DURING ALL PHASES OF CONSTRUCTION. 10) THE CONTRACTOR IS RESPONSIBLE FOR CLEARING AND DISPOSING OF ANY MAN-MADE MATERIALS OR OTHER DEBRIS THAT LIE WITHIN THE CONSTRUCTION LIMITS, AS REQUESTED AND APPROVED BY THE PROJECT ENGINEER. DEBRIS AND SEDIMENT REMOVED FROM THE PROJECT AREA MUST BE TAKEN TO A PERMITTED FACILITY. 11) THE CONTRACTOR IS RESPONSIBLE FOR ENSURING THAT THE STREETS IN FRONT OF THE CONSTRUCTION SITE WILL BE KEPT CLEAN AT ALL TIMES. IF NEEDED TO ACCOMPLISH THIS A WASH STATION MAY BE REQUIRED. 12) ALL WORK SHALL BE COMPLETED "IN THE DRY" WITH THE USE OF PUMP AROUND OR SIMILAR SUITABLE METHOD OF WATER DIVERSION. SEEDING NOTES: SOIL PREPARATION ALL DISTURBED AREAS SHALL BE DRESSED AS DIRECTED. ANY COMPACTED AREAS WILL BE LOOSENED TO A DEPTH OF 6-8 INCHES. THE TOP 3 INCHES SHALL BE PULVERIZED TO PROVIDE A UNIFORM SEEDBED AFTER THE ADDITION OF SOIL AMMENDMENTS. INCIDENTAL GRADING SHALL NOT CONSTITUTE SUBSTANTIAL OR SIGNIFICANT PROGRESS IN CONSTRUCTION ACTIVITY. AS A PART OF PERMANENT SEEDING, MAINTENANCE MAY BE REQUIRED TO MAINTAIN VEGETATION FOR 12 MONTHS (INCLUDING MOWING AND WATERING). THIS MAINTENANCE SHALL BE CONSIDERED A PART OF ESTABLISHING PERMANENT GROUND COVER. RE -SEED ALL DISTURBED AREAS AND SLOPES AS DIRECTED. GEOTECHNICAL NOTES: 1) THE CONSTRUCTION OF THE EMBANKMENT SHALL BE DONE UNDER THE OBSERVATION OF A QUALIFIED GEOTECHNICAL ENGINEER, WHO IS REGISTERED AS A PROFESSIONAL ENGINEER IN THE STATE OF NORTH CAROLINA. 2) TEST OF THE DEGREE OF COMPACTION OF FILL SHALL BE PERFORMED AS A PART OF CONSTRUCTION OBSERVATION FOR THE CONSTRUCTION OF THE DAM. TEST SHALL BE CONDUCTED IN CONCURRENT WITH THE INSTALLATION OF THE COMPACTED FILL AND THE CONTRACTOR SHALL COORDINATE THE CONSTRUCTION OF THE DAM SO THAT THE TESTING CAN BE COMPLETED. IN ALL FILL AREAS OF THE BERM, A SOIL COMPACTION TEST SHALL BE CONDUCTED EACH 2,500 SF PER VERTICAL FOOT OF FILL. SHOULD THE RESULTS OF THE TEST INDICATE THAT THE SPECIFIED DEGREE OF COMPACTION HAS NOT BEEN OBTAINED; THE PORTIONS OF THE DAM REPRESENTED BE SUCH TESTS SHALL BE REBUILT. ALL PORTIONS OF THE DAM SHALL ACHIEVE THE SPECIFIED MINIMUM DEGREE OF COMPACTION. 3) THE GEOTECHNICAL ENGINEER SHALL OBSERVE ALL ASPECTS OF THE CONSTRUCTION OF THE DAM (I.E., PREPARATION OF THE FOUNDATION, INSTALLATION OF THE CUT -OFF -TRENCH, INSTALLATION OF THE PRIMARY OUTLET PIPE, INSTALLATION OF THE INTERNAL DRAINAGE, INSTALLATION OF COMPACTED FILL, ETC.). THE FREQUENCY OF OBSERVATION AND TESTING MUST BE SUFFICIENT FOR THE GEOTECHNICAL ENGINEER TO STATE, IN HIS/HER PROFESSIONAL OPINION, THAT THE SPECIFIC ITEMS OBSERVED AND TESTED WERE INSTALLED IN ACCORDANCE WITH THE APPROVED CONSTRUCTION DRAWINGS AND SPECIFICATION. REFERENCE IS MADE TO SECTIONS 4.1 TO 4.5 REGARDING GEOTECHNICAL CERTIFICATION OF THE DAM EMBANKMENTS. DAM CONSTRUCTION NOTES: 1) CONTROLLED FILL, AS SPECIFIED BY THE GEOTECHNICAL ENGINEER, IN THE DAM EMBANKMENT AND CUTOFF TRENCH SHALL BE PLACED IN 6-INCH LOOSE LAYERS AND SHALL BE COMPACTED TO A DENSITY OF NO LESS THAN 95% OF THE STANDARD PROCTOR MAXIMUM DENSITY AT A MOISTURE CONTENT OF + OR - TWO PERCENTAGE POINTS OF THE OPTIMUM MOISTURE CONTENT IN ACCORDANCE WITH ASTM D698. FILL PLACED WITHIN 10-FEET OF THE CENTERLINE OF THE PRINCIPAL OUTLET PIPE SHALL BE PLACED IN 3-INCH LOOSE LAYERS AND SHALL BE COMPACTED TO A DENSITY OF NO LESS THAN 98% OF THE STANDARD PROCTOR MAXIMUM DENSITY. 2) ALL VISIBLE ORGANIC DEBRIS SUCH AS ROOTS AND LIMBS SHALL BE REMOVED FROM THE FILL MATERIAL PRIOR TO COMPACTION TO THE REQUIRED DENSITY. SOILS WITH ORGANIC MATTER CONTENT EXCEEDING 5% BY WEIGHT SHALL NOT GE USED. STONES GREATER THAN 3-INCH (IN ANY DIRECTION) SHALL BE REMOVED FROM THE FILL PRIOR TO COMPACTION. 3) FILL MATERIAL PLACED AT DENSITIES LOWER THAN SPECIFIED MINIMUM DENSITIES OR A MOISTURE CONTENTS OUTSIDE THE SPECIFIED RANGES OR OTHERWISE NOT CONFORMING TO SPECIFIED REQUIREMENTS SHALL BE REMOVED AND REPLACED. 4) ANY FILL LAYER THAT IS SMOOTH DRUM ROLLED TO REDUCE MOISTURE PENETRATION DURING A STORM EVENT SHALL BE PROPERLY SCARIFIED PRIOR TO THE PLACEMENT OF THE NEXT SOIL LIFT. 5) SURFACE WATER AND STREAM FLOW SHALL BE CONTINUOUSLY CONTROLLED THROUGHOUT CONSTRUCTION AND THE PLACEMENT OF CONTROLLED FILL. 6) FOUNDATION AREAS MAY REQUIRE UNDERCUTTING OF COMPRESSIBLE AND /OR UNSUITABLE SOILS IN ADDITION TO THAT INDICATION ON THE PLANS. ALL SUCH UNDERCUTTING SHALL BE PERFORMED AT THE DISCRETION OF THE GEOTECHNICAL ENGINEER AND SHALL BE MONITORED AND DOCUMENTED. IN NO CASE SHALL THERE BE AN ATTEMPT TO STABILIZE ANY PORTIONS OF THE FOUNDATIONS SOILS WITH CRUSHED STONE. THE GEOTECHNICAL ENGINEER SHALL PROVIDE THE DESIGN ENGINEER WITH A COPY OF ALL FIELD REPORTS THAT REFERENCE THE COMPACTION OR UNDERCUT REQUIRED TO CONSTRUCT THE BMP. 7) TREATMENT OF SEEPAGE AREAS, SUBGRADE PREPARATION, FOUNDATION DEWATERING AND ROCK FOUNDATION PREPARATION (I.E., TREATMENT WITH SLUSH GROUTING, DENTAL CONCRETE, ETC.) MAY BE REQUIRED AT THE DISCRETION OF THE GEOTECNICAL ENGINEER. ALL SUCH ACTIVITIES SHALL BE CLOSELY MONITORED AND DOCUMENTED BY THE GEOTECHNICAL ENGINEER. 8) FILL ADJACENT TO THE RISER AND PRINCIPAL OUTLET PIPE SHALL BE PLACED SO THAT LIFTS ARE AT THE SAME LEVEL ON BOTH SIDES OF THE STRUCTURES. 9) EARTHWORK COMPACTION WITHIN 3-FEET OF ANY STRUCTURES SHALL BE ACCOMPLISHED BY MEANS OF HAND TAMPERS, MANUALLY DIRECTED POWER TAMPERS, OR PLATE COMPACTORS OR MINIATURE SELF-PROPELLED ROLLERS. 10) COMPACTION BY MEANS OF DROPPING WEIGHTS FROM A CRANE OR HOIST SHALL NOT BE PERMITTED. 11) HEAVY EQUIPMENT SHALL NOT BE ALLOWED TO PASS OVER CAST -IN -PLACE STRUCTURES (INCLUDING THE CRADLE) UNTIL ADEQUATE CURING TIME HAS ELAPSED. 12) TO RE-ESTABLISH VEGETATION AFTER CONSTRUCTION, A 2- TO 3-INCH LAYER OF TOPSOIL SHALL BE PLACED ON THE DISTURBED EMBANKMENT SURFACE AND THE AREA SEEDED AND MULCH OR HYDROSEEDED. ANTI -SEEPAGE COLLARS NOTES: 1) BECAUSE THE CONTACT BETWEEN AN EMBANKMENT PENETRATING CONDUIT AND THE SOIL IN THE EMBANKMENT IS THE AREA MOST SUSCEPTIBLE TO PIPING (E.G., INTERNAL EROSION OF EMBANKMENT SOIL), SPECIAL ATTENTION MUST BE GIVEN TO THE DESIGN OF THE PRINCIPAL OUTLET PIPE. DEPENDING ON THE DAM HEIGHT AND FACILITY TYPE (E.G., A WET POND VS. A DRY POND), THE INSTALLATION OF CONCRETE ANTI -SEEPAGE COLLARS SHALL BE PROVIDED AND CONSTRUCTED UNDER THE OBSERVATION OF A QUALIFIES GEOTECHNICAL ENGINEER, WHO IS REGISTERED AS A PROFESSIONAL ENGINEER IN THE STATE OF NORTH CAROLINA. THE GEOTECHNICAL ENGINEER MUST HAVE EXPERIENCE IN THE DESIGN AND CONSTRUCTION OF SMALL EARTH DAMS. 2) UNTILITY CONDUITS OTHER THAN THE PRINCIPAL OUTLET PIPE SHALL NOT BE PERMITTED TO PENETRATE THE EMBANKMENT OR THE EMBANKMENT FOUNDATION. THESE UTILITY CONDUITS SHALL BE ROUTED AROUND THE EMBANKMENT AND FOUNDATION AREAS. 3) CONCRETE IN THE ANTI -SEEPAGE COLLARS SHALL HAVE A 28-DAY COMPRESSIVE STRENGTH OF 4,000-PSI . A COMPRESSIVE STRENGTH TESTS SHALL ALSO BE PERFORMED TO ENSURE COMPLIANCE WITH THESE SPECIFICATIONS. THE CONCRETE ANTI -SEEPAGE COLLARS SHALL BE PLACED MONOLITHICALLY, SHALL BE WORKED WITH HAND EQUIPMENT UP UNDER THE PRINCIPAL OUTLET PIPE, SHALL BE CONSOLIDATED PRIMARILY BY INTERNAL VIBRATION AND SHALL BE FINISHED "ROUGH" SO AS TO ACHIEVE A MORE ADEQUATE BOND BETWEEN THE CONCRETE IN THE ANTI -SEEPAGE COLLARS AND THE FIRST LIFT OF STRUCTURAL FILL. CRADLES CONSTRUCTED OF FLOWABLE FILLS, SLURRIES OF GELS WILL NOT BE PERMITTED. 4) THE CONTRACTOR SHALL CONTACT THE PROJECT ENGINEER WHEN INSTALLING THE CONCRETE ANTI -SEEPAGE COLLARS TO OBSERVE THE INSTALLATION. THE ENGINEER MUST WITNESS AND PHOTOGRAPH THE INSTALLATION OF THE CONCRETE ANTI -SEEPAGE COLLARS FOR AS -BUILT PURPOSES.
