4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
Sunnybrook Estates is an existing subdivision constructed around the year 2000. The subdivision was permitted by the City of Raleigh to utilize an existing farm pond as a wet pond
stormwater device. The watershed is developed with single family and townhomes. This project includes modifications to the pond and dam including installation of a riser, outlet pipe,
grading of a littoral shelf, and dam grading.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past? *
Yes No Unknown
4f. List the total estimated acreage of all existing wetlands on the property:
0
4g. List the total estimated linear feet of all existing streams on the property:
236
4h. Explain the purpose of the proposed project: *
The purpose of the project is to install a proper riser structure and outlet pipe capable of establishing a permanent pool and managing stormwater runoff peak flows. Additionally, tree
removal and grading will take place on the dam and grading in the pond to form a littoral shelf. Cumulatively, these modifications to the pond and dam will bring the pond into compliance
with City of Raleigh stormwater wet pond requirements.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The project will include a construction entrance from Sunny Cove Dr. The construction entrance will access the pond dam. Trees on the dam will be removed and the water level in the
pond lowered. An outlet pipe will be installed in the dam and a riser structure in the pond. A new outlet dissipater will be installed at the outlet location. The dam will be lowered slightly and
the spoils will be used to grade a littoral shelf around the pond. The riser structure will managed all flows and no spillway over the dam is proposed. The project will utilize dump trucks and
excavators.
5. Jurisdictional Determinations
5a. Have the wetlands or streams been delineated on the property or proposed impact areas?*
Yes No

E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The limits of disturbance have been reduced to only the areas necessary to complete the proposed modifications to the existing stormwater facility.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
The construction entrance is situated at an upland location out of stream buffers. This limits the impact to buffers and streams.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
Yes No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
All impacts are temporary. No change of use is proposed, the existing wet pond is receiving some improvements.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
la. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
Yes No
What type of SCM are you providing?
Level Spreader
Vegetated Conveyance (lower SHWT)
Wetland Swale (higher SHWT)
Other SCM that removes minimum 30% nitrogen
Proposed project will not create concentrated stormwater flow through the buffer
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250? *
Yes No
2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)?
Yes No
2c. Does this project have a stormwater management plan (SMP) reviewed and approved under a state stormwater program or state -approved local government stormwater program?
Yes No
N/A - project disturbs < 1 acre
3. Stormwater Requirements
3a. Select whether a completed stormwater management plan (SMP) is included for review and approval or if calculations are provided to document the project will not cause degradation of
downstream surface waters.*
