P.O. Box 929 Monroe, NC 28111-0929 SUBJECT: Pre -Construction Conference Minutes A pre -construction conference was held Monday, August 29, 2022 for the above referenced project. Bridge Maintenance Supervisor Michael Aman NCDOT — Division Bridge Program Manager Garcy Ward NCDEQ/DWR — Environmental Specialist Ronnie Melker Dellinger — Vice President General Items Mr. Bullard opened meeting with introductions by all attendees. Mr. Bullard has received all the initial submittal documents from Contractor with exception of progress schedule and SAFs. Mr. Melker said those documents will be submitted soon. The estimate end date of the 15t' was agreed upon by Mr. Melker and Mr. Bullard. The Resident Engineer for this project is Aaron Bullard. Contract Review Floating Availability Date: August 8 to August 31, 2022 Overall Completion Date 90 days after completion of ICT #1 Liquidated Damages: $200.00 per calendar day ICT #1 (All work except permanent vegetation establishment) Floating Availability Date: Same as above Completion Date: 150 days after Date of Availability Liquidated Damages: $700.00 per calendar day Contractor plans to start on Tuesday, September 6, 2022. Mr. Melker has contacted Traffic Services for the detour installation on that date. Contractor will email a schedule to Mr. Bullard with a target date for completion. Project has the Permanent Vegetation Establishment provision. Contract DBE goal was 5% and was met at 15.8% with Everett's Lot Clearing & Grading. Contractor will have other DBEs — Garris Paving and a 4 D Construction. Mr. Bullard instructed contractor to be sure to enter all payments into the Payment Tracking System each month, for both committed and non - committed DBEs. Contractor will not be using any Unmanned Aircraft Systems on this project. Mr. Bullard reminded the Contractor to be aware of Equipment Idling Guidelines. Subsurface Information is available for the structure portions, but there is no information for the roadway portions. NCDOT forces will do the offsite detour and final pavement markings after everything is done. Contractor has contacted Mr. Jim Evans with Traffic Services. Mr. Harrell has contacted EMS and schools and should be good to proceed on September 6. Contract has a twelve-month guarantee on major items/components and workmanship. Pavement along the transverse centerline of all caps shall be saw cut to a depth of 3/4"f, cleaned, and filled with an approved asphalt sealant. Apply an approved epoxy protective coating to the tops of all caps and to the exterior faces of all exterior cored slab units with a Type 4A epoxy coating. Clearing and Grubbing will be Method II. Pre -Construction Mins (DB00539) August 29, 2022 Page 3 of 4 Mr. Bullard stated there was an addendum that added the provision for bridge approach fills. It will be the standard modified approach fill for these smaller cored slab bridge projects. It's the last line item in the contract. Automated Machine Guidance will not be used on this project. Type III guardrail end units shall come off the APL. Mr. Bullard requested Acceptance Letter and Working Drawings prior to installation. Bullington will be doing the guardrail CenturyLink, Suddenlink, and North Lenoir Water Corporation have completed their work so there should be no utility conflicts remaining. A Supplemental Agreement was added for Response for Erosion Control. This was omitted from original contract. Borrow source has not been decided yet. Contractor will advise. Mr. Ward reviewed the DEQ Water Quality Certification. Mr. Ward questioned if Surveys for Neuse River Water Dog and Carolina Madtom had been done. Mr. Aman confirmed these surveys were done on January 28, 2022. Contractor will send copies to Mr. Garcy Ward and to Mr. Tom Steffens. Mr. Ward stated Neuse buffer rules are in effect. Because of small buffer impact, Buffer Authorization was not required. Mr. Bullard noted that the Buffer Permit Drawings were not included in the contract. Mr. Aman will send these drawings to Mr. Bullard. Mr. Bullard reviewed the Title VI contract requirements, specifically the posting of NCDOT's Notice of Non-discrimination and the Contractor's EEO Policy on your job board. Include all Title IV clauses in all contracts and subcontracts. Mr. Bullard will need one DBE and one non -DBE of subcontract documents for verification. Plan Review Mr. Aman noted that on plan sheet 4, there is a note regarding minimizing disturbance of trees at the Northeast quadrant. Mr. Blankenship asked Contractor to be mindful of the trees in the easement and leave as many trees undisturbed as possible. It was noted that the allowable buffer impacts only extend to the toe of fill and do not encompass the construction easements, rendering the easements in the buffer zones unusable. Mr. Harrell reached out to the schools about TMP Plan note to create a place for school bus turn around. He has not had a response from the schools yet but will advise once he hears back. Mr. Harrell mentioned that there is a Roadway item for Class II Rip Rap (220 tons) in addition to the Structure item of Class II Rip Rap (125 tons). The Structure item is clearly for the End Bent slope protection, but the plans do not specify the location of the Roadway item. Mr. Bullard will review the Calculation of Quantities to try and find an answer. Mr. Bullard noted that the Bridge Approach Slab plan sheet does not show the layer of geotextile to be installed 1 foot below the top of the approach fill. Ensure that gets installed. Additional Comments Project survey control has been verified per Mr. Harrell. Pre -Construction Mins (DB00539) August 29, 2022 Page 4 of 4 East Group will be doing the surveying for contractor. Contractor Project Superintendent will be Chris Evans and General Superintendent will be Chris McCray. A copy of the attendance roster will be attached to these minutes for information. Meeting was adjourned by Mr. Bullard. These minutes are prepared as documented. Any discrepancies with the content of these minutes should be submitted in writing to this office.
