PO Box 9296 Greensboro, NC 27429 SUBJECT: Notice of Violation (NOV-2022-PC-0475) Compliance Evaluation Inspection Permit No. WQ0013808 Summerfield Shopping Center WWTF Guilford County Dear Mr. Gaster: On August 23, 2022, Division of Water Resources staff Caitlin Caudle and Rebecca Chandler conducted a compliance evaluation inspection. Chad Leinbach, ORC, was present during the entire inspection. A review of records, treatment facilities, storage facilities, and irrigation fields was completed. As a reminder, the current permit expires July 31, 2023. A permit renewal application must be submitted no later than six months before the expiration of the current permit. The following violation was noted: 1. Monthly monitoring reports were not submitted as required by Permit Condition 111.7 since December 2021. Monthly reports for December 2021, January, February, and March 2022 were addressed in correspondence dated July 19, 2022. Monthly reports for April 2022 were addressed in correspondence dated August 15, 2022. Monthly reports for May and June 2022 were submitted late per Permit Condition 111.7. This is a violation of the subject permit. Moving forward, submit all monthly reports no later than the last day of the month following the reporting month. An updated NDAR-1 form was provided with correspondence dated August 4, 2021. To date, the new NDAR-1 form has not been used. An updated NDMR form is enclosed. Moving forward, please use the most recent reporting forms. At the time of inspection, the piping associated with the irrigation pump closest to the doors was leaking (Pump B). Mr. Leinbach stated that the check valve likely needs to be repaired as the pump that was leaking was not being used to irrigate zones during the inspection. This same leak was noted in correspondence dated June 22, 2021. Per Permit Condition 111.1 "the facility shall be properly maintained and operated at all times..." Please repair this leak as soon as possible. B. A review of January to July 2021 monitoring reports showed that the 12-month floating total in Zone 3 exceeded the limit established in Attachment B in the months of June and July 2022. Irrigation to Zone 3 has been turned off until the 12-month floating total has returned to compliance. Exceeding the 12-month floating total permitted in Attachment B is a violation of the subject permit. The following items of concern were noted: • The septic tank behind Walgreens did not have a lock on the lid at the time of inspection. Per Permit Condition 111.9 "public access to the irrigation sites and wastewater treatment facilities shall be prohibited." Please replace the lock on this septic tank as soon as possible. • There is a small tear in one corner of the storage pond liner. Per Permit Condition 111.1 "the facilities shall be properly maintained and operated at all times." Please take measures to repair the tear and prevent it from growing larger. Other observations: • No seeping was visible from the sand filter junction box. There was standing water next to the liner but was likely from 3.5" of rain the previous weekend. • No vegetation was present in the sand filter. • Hauling records for the grease trap and septic tanks were present. • There did not appear to be a blockage between the wet weather storage pond and the wet well. The wastewater level in the wet well visually was the same level as the storage pond at the time of inspection. • At the time of inspection, the highwater alarm in the storage pond wet well was silenced and when turned back on it was activated via float. The float tree arrangement may need to be revisited as the wet well and storage pond are hydraulically connected. If the staff gauge is correct, the highwater alarm was activated when freeboard was <2ft. • No trees were in the wetland cells. • The septic tanks at Food Lion needed pumping. Mr. Leinbach stated he would call the facility manager to confirm that a pumping event was scheduled, and if not, he would schedule one. Page 2 of 7 Permit: WQ0013808 Owner - Facility: Kotis Properties Inc Inspection Date: 08/23/2022 Inspection Type : Compliance Evaluation Reason for Visit: Routine • Z1 was in good condition with only one "geyser" noted while the zone was being irrigated. Z2 was also in good condition. The few drip lines in the wet area are still turned off. These drip lines will likely not be returned to use. Z3 is currently turned off as the 12-month floating total was exceeded in June. Mr. Leinbach has reduced the volume dosed during each cycle for this zone. There was no runoff at the bottom of Z1. The valve box closest to the treatment building did have standing water in it. • The soil report recommended that each zone have about 1 ton of lime applied. The crop listed was fescue hay. The irrigation zones are planted in trees, which can tolerate a lower pH than the reported fescue hay. The wastewater produced is low strength, currently there are no concerns for soil pH. • Both septic tanks have filters. • The sand filter is currently on a timer and was being dosed every 15 minutes. Mr. Leinbach was unsure of how long the sand filter is dosed. • The highwater alarm was working at the time of inspection. The float for the alarm was hooked over the ladder. The float was removed from the ladder. Visually, there appeared to be a large amount of wastewater in the tank, approximately three foot of head space was available in the tank. Mr. Leinbach stated that he was currently operating the system to stack water up in the wetland cells, and as a result in the recirculating tank. The arrangement of the float tree in the recirculating tank may need to be revisited. • The grease trap was not reviewed. • Tablets were present in the tube of the contact chamber. • Flow is reported based on water meters. • The red float will transfer wastewater between the storage pond and the irrigation wet well when the float is down. • The green float will shut off wastewater from entering the treatment building and stack up water in the wetland cells • When the system is running based on floats, the yellow float will engage the irrigation pumps when up. • Currently the irrigation system is working on a timer. Z1 and Z2 are irrigated every four hours with 500 gallons irrigated. The system is connected to a rain sensor and is not operational when raining.
