Please find attached the Stormwater Outfall Discharge Monitoring Reports for Columbia Forest
Products located in Old Fort, North Carolina (Columbia Carolina). Columbia Carolina operates
under Certificate of Coverage Number NCG210136. Samples were collected on 5/26/2022 and
analytical results were received 6/9/2022.Outfall #8 was above benchmarks for COD. A sormwater
inspection was conducted within two weeks and identified the bark/mulch pile as the likely source
of COD at Outfall #8. This inspection was followed by a Tier I Response Form and appropriate
Corrective Actions, including the removal of the bark/mulch piles nearest to the ditch leading to
Outfall #8, which started in June 2022. Columbia Forest Products is working to remove the
bark/mulch piles entirely by the first quarter of 2023. Outfall #8 remains in Tier Two and will
continue to be sampled monthly until three consecutive lab results are below benchmarks.
